Xxxx Leave Donation. 1. To be eligible to receive donated days, an individual must be a member of the Sick Leave Bank. (Revised 2003) 2. Upon a request from the Association, an employee may donate two (2) days of his/her accumulated sick leave to another employee for personal health issues (physical and mental) and /or health issues of family and/or significant others. The Association may request donations for an employee if the initial request does not generate the maximum number of days allowed under this sick leave donation article. (Revised 2001) 3. An employee may use up to twenty (20) donated days in a school year for the illness of herself/himself, family, and/or significant others providing other employees have donated sufficient days. (Revised 2001, 2003) 4. The donated days will be administered by the Association. The Committee will be composed of members appointed by the Association to include the GEA Executive Board. The following conditions will govern the granting of sick leave days from the Committee : (Revised 2000. 2004) a. Donation forms will be available from the Association. b. Donated sick leave days will be granted only when an employee has used all of his/her accrued and projected sick leave days and discretionary days. If an employee accesses Sick Leave Bank and/or sick leave donation. a maximum of one-day sick leave must be donated to the Sick Leave Bank at the beginning of his/her next contract year. This does not eliminate the employee 's responsibility to donate as called for by the Sick Leave Bank Committee. If the employee is not otherwise a member of the Sick Leave Bank, this donation does not entitle him/her to membership. (Revised 1995, 1999) c. Not more than twenty (20) Donated Sick Leave days may be used by any one employee in any school year. (Revised 2001, 2003) d. A doctor's statement specifying the nature of the illness, the dates and medical service to the employee's family will be requested. e. Donated days will be used in order of receipt and turned in to the Finance Department by the second Friday of the month. (Revised 1995) 5. The Association will report the status of the donated sick leave days in writing to the Finance Department for each occurrence. (Revised 1995) 6. The Association and the Sick Leave Donation Committee assume the full responsibility for the control of the donated sick leave, and will be responsible for any actions as a result of the Committee's operation. Xxxx Leave Donation. The Federation will develop and disseminate the criteria for qualifying for sick leave donation. 1. In the event an employee needs sick leave time in excess of those provided pursuant to this Agreement, additional sick leave may be provided by contribution of accumulated but unused sick leave by any other employee in the bargaining unit. The sick leave donation may not reduce his or her accumulated sick leave to less than ten (10) days. Donated sick leave may be used by a bargaining unit member in the same manner that accumulated sick leave may be used. 2. An employee shall not be eligible to receive donated sick leave time until that employee has exhausted all of his or her accumulated leave (sick, personal, vacation). While receiving donated sick leave, an employee may not receive pay or compensation from any other plans in which the District participates, either in full or in part. For purposes of this section, District insurance contributions are not considered compensation. 3. Under no circumstances will an employee be allowed to access or receive more than twenty (20) donated days per school year. 4. For purposes of this section, a day shall be defined as the number of hours that the donor or recipient is normally scheduled to work. 5. An employee seeking sick leave donations must complete the appropriate form which the Federation will provide.
Xxxx Leave Donation. Any employee whose sick leave accumulation is forty-one (41) days or more may relinquish no more than one (1) sick day to a single employee annually whose sick leave has been exhausted. Any employee may donate a maximum of two (2) days per year. This donation may only occur if the employee with depleted sick leave is seriously ill or seriously injured and only after the employee has been subject to a wage deduction for a minimum of seven (7) workdays. Up to twenty (20) days may be donated for maternity leave or adoption and may be used once during their Rockford Public School’s career. Should employees desire to donate, a written letter shall be submitted to the Superintendent of Schools or his/her designee by the union president requesting a review of all aspects of the request. A committee consisting of the Superintendent of Schools or his/her designee and the union president shall communicate to review the merits of the request.
Xxxx Leave Donation. 1. Upon request, any employee who is suffering from a catastrophic illness or injury as defined in Camden County Policy Number 52.1 effective 12/13/01 requiring prolonged absence from work shall be eligible to receive a maximum of 90 days of donated sick leave from such sick days in the leave bank accumulated by contributions from leave donors provided the employee has completed at least one year of service and has exhausted all accrued sick, vacation and personal leave. A catastrophic illness or injury shall be understood as a condition which has required the employee to be hospitalized and which requires a period of treatment or recuperation, as a result of which the employee is expected to be unable to work for at least 90 days, based on medical prognosis. Request for donated sick leave must be made prior to exhaustion of medical insurance. Upon the employee’s return to work, any donated leave that remains unused shall remain in the accumulated sick day’s bank. Individual employees may donate up to 15 sick days each but must retain a balance of at least 30 sick days. No employee shall be subject to coercion of any kind in connection with the donation of sick leave. Nor may any employee be paid any remuneration for making a donation. 2. Retirees may donate as many sick days as they want upon retiring from the Board to the CWA sick bank in accordance with limitations imposed by the State. 3. In the event the IRS shall determine Article 9 section F. to be unqualified the parties will then meet to review how the spirit of the agreement could be legally implemented.
Xxxx Leave Donation. Sick Leave Donation Application Process 1. Employees who request sick leave donation must apply using the form available in Human Resources. 2. The requirements for consideration are as follows: a. An employee has used all available paid leave days and paid vacation days and; b. Is not able to return to work for medical reasons due to a serious illness or injury that prevents the employee from performing the duties of his or her job for an extended period of time (ten (10) days or more) or; c. A condition that qualifies for short-term disability or long-term disability (but the qualification period has yet to run) and therefore, needs donated days, the employee may submit a donation request for sick leave.

An employee who experiences a major incident in his/her life or the life of immediate family and exhaust accrued sick leave shall have the option to obtain thirty (30) days per school year from the sick leave bank.
