LEAVE PROGRAM. The Catastrophic Leave Bank (C.L.B.) is a “bank” of donated sick leave days which may be requested for use by Unit Members who are suffering from a catastrophic illness or injury, once they have exhausted all full-paid leaves.
5.19.1 A Unit Member is eligible to use Catastrophic Leave when the following conditions are met:
a) The Unit Member requesting use of the leave has donated leave to the program pursuant to the minimum contribution (see 5 below) requirements set forth in this procedure prior to requesting leave;
b) The Unit Member has submitted a written request that includes a physician’s verification of a severe or incapacitating illness or injury that is expected to continue for an extended period of time and prevents the Bargaining Unit Member from performing his/her duties;
c) The Unit Member will not receive payment for the time absent due to illness or injury from Workers’ Compensation (C.W.C.I.A.);
d) The Unit Member has exhausted all balances in all full-paid leave categories;
e) The Unit Member has not withdrawn more than forty (40) days of sick leave per catastrophic illness or injury;
f) There is sufficient leave in the C.L.B.
5.19.2 Enrollment in Catastrophic Leave Bank
a) Current Unit Members: An annual open enrollment period will occur each March; however nothing precludes any C.L.B. member from donating to the bank at any time.
b) New Hires: Newly hired Unit Members may donate one day of sick leave of the C.L.B. immediately upon hire. After the initial enrollment, the Unit Member must maintain the required balance in order to donate.
LEAVE PROGRAM. Permanent employees represented by the SEIU Public Works Unit, may be eligible to receive donations of paid leave, to be included in the recipient employee’s sick leave balance if he/she has suffered a catastrophic illness or injury which is defined as a serious medical condition considered to be terminal, a major physical impairment, or a family medical emergency (defined as a catastrophic illness or injury of a spouse, registered domestic partner, child or parent residing in the employee’s household), subject to the following conditions: • The recipient employee, recipient employee’s family, or other person designated in writing by the recipient must submit a request to the City Administrator or designee. • The recipient employee is not eligible so long as he/she has paid leave time available; however, the request may be initiated prior to the anticipated date that all leave balances will be exhausted. • The recipient employee must provide a medical verification which meets the criteria above and a prognosis. • Donations may be made in whole hour increments, and are irrevocable. The donor employee may donate vacation up to any amount, but the City may require that the donor employee retain 40 hours of vacation leave. Sick leave may be donated up to 24 hours in whole hour increments. Donations may be made from any individual employed by the Public Works Department. • Time donated will be converted from the type of time donated to sick leave and credited to the recipient employee’s sick leave balance on an hour-for-hour basis and shall be paid at the rate of pay of the recipient employee. • Time donated in any pay period may be used in the following pay periods. No retroactive donations will be permitted. • Eligibility for this program requires recommendation by the Public Works Director and approval by the City Administrator.
LEAVE PROGRAM. The County will provide for a voluntary Catastrophic Illness/Injury Leave Program as follows:

LEAVE PROGRAM. 282. Covered employees may participate in the catastrophic leave program administered by the City and County of San Francisco pursuant entirely to its provisions.
LEAVE PROGRAM. A. The City agrees to permit employees within the bargaining unit to contribute a portion of their accrued leave credit to another employee when such employee is on an approved leave of absence due to a verifiable illness or injury caused by a physical or mental impairment. Catastrophic as used in this provision refers to an unforeseen or sudden unexpected illness or injury requiring immediate attention.
B. The City also agrees to permit employees to contribute a portion of their accrued vacation and/or compensatory time to another employee when such employee is on an approved leave of absence due to the verifiable illness or injury of the employee’s spouse, domestic partner, parent or child. Sick leave may not be donated when the catastrophic leave fall under this provision (17.05 B). For such transfer to take place the following conditions shall apply:
1. The Human Resources Department shall review all requests from the receiving employee to determine eligibility for catastrophic leave prior to the receiving employee or the employee’s Department requesting donations.
2. Such illness or injury is defined as an unforeseen or sudden, unexpected illness or injury requiring immediate attention. Pregnancy is excluded from this provision; however, injuries or illnesses resulting from a pregnancy may be covered under this provision with a statement from the employees treating physician as the final authority.
3. The receiving employee has exhausted all accrued leave and is therefore facing financial hardship.

5. The receiving employee must be on an approved continuous leave of absence for over 40 hours due to the illness or injury. A statement from the employee’s or family members treating physician is required.
6. Contributing employee must have at least 96 hours remaining after such contribution and the leave application rate will be based on the contributing employee’s dollar value which will be adjusted proportionally to the receiving employee’s rate.
7. The transfer must be in whole hour increments. The amount of the donations credited each pay period shall be in accordance to the receiving employee’s needs.

LEAVE PROGRAM. The Catastrophic Leave Program is for employees who have exhausted all accrued leaves due to a serious or catastrophic illness, injury, or condition. The Catastrophic Leave Program allows employees to donate time to any qualifying employee so that he/she can remain in a paid status for a longer period of time, thus partially ameliorating the financial impact of the illness, injury or condition.
LEAVE PROGRAM. The District and the Association agree to the establishment of a Classified Unit Member Catastrophic Leave Program (Catastrophic Leave Program). The intent of this Catastrophic Leave Program is to provide additional financial protection to those unit members who incur a period of prolonged illness/injury or hospitalization.
LEAVE PROGRAM. The granting of leave shall be in writing and shall be approved by the Sheriff, or designee, prior to the leave being taken, except in the case of an emergency. Said leave shall be granted if the member has sufficient time accrued, unless this would create undue hardship on the remaining scheduled personnel, as in the case of too many other personnel already scheduled for leave time. When prior approval for leave cannot be obtained by the member due to an emergency, the Shift Sergeant will be notified, and a leave request filed immediately.
LEAVE PROGRAM. The provisions of this ARTICLE shall apply only to regular, full-time employees in the classified service. Employees appointed to certain grant funded positions as designated by the Anaheim City Manager under Section 22.8 and newly hired probationary employees shall be excluded from the provisions of this ARTICLE.
