PTO Use. PTO may be taken after 90 days of employment. The use of PTO will be voluntary if an employee is on FMLA/OFLA approved leave and has less than two times (2x) their weekly FTE accrued. PTO shall also be voluntary when an employee is sent home on a mandatory absence (MA).

PTO Use. PTO may be taken after ninety (90) days of employment. However, employees may not have a negative balance in their PTO accounts. Any leave taken prior to ninety (90) days of employment must be approved by management and will be considered unpaid time.
PTO Use. 1. Employees may begin to request, schedule, and use accrued PTO hours immediately upon accrual. There is no waiting period. Employees may receive payment for approved time off as long as they have a sufficient amount of PTO time in their bank. 2. During the first four (4) months of employment, newly hired employees assigned to departments that are not staffed on holidays will be allowed to have a temporary negative balance in their PTO bank and will be paid for the holiday if the employee was scheduled to work and the employee requests to use PTO. However, the negative balance is not to exceed the number of scheduled hours attributed to the holiday. 3. With any scheduled time off, the supervisor’s approval is required in advance. 4. Because staffing needs and workloads vary, some departments may have specific procedures which outline PTO scheduling parameters, such as the specified period of time required for requesting vacations and other personal time off. 5. PTO hours are to be used for requested time off and for time off on recognized holidays, except as otherwise required by law or policy. Supervisors may in advance, at their discretion, approve unpaid scheduled time off for non-exempt employees not to exceed sixteen (16) hours for 8-hour, twenty (20) hours for 10- hour, and twenty-four (24) hours for 12-hour employees per calendar year. Employees must submit their request for unpaid time off at least 48 hours prior to the scheduled shift for which they are requesting to take unpaid time off. 6. Accrued PTO may be used at a rate not to exceed the employee’s FTE status each pay period as shown in the Human Resources Information System, except in cases when the employee had used PTO for a scheduled workday and then was asked to work additional hours or shifts. 7. Any accrued, unused PTO hours will be paid out upon termination of employment. If termination is the result of Inter- Affiliate Employment, refer to the Inter-Affiliate Employment Policy. 8. Accrued PTO may not be used for an employee’s last day of employment, unless applicable law provides otherwise. Employees must work their last day of employment. Inter- Affiliate transfers may use up to one week (40 hours) of accrued PTO to bridge their last day of actual work at the departing affiliate and the first day of employment at the hiring affiliate.
PTO Use. An employee may utilize accrued PTO on a daily basis provided the employee notifies the Employer at least two (2) hours prior to the start of the employee’s shift. Requests for PTO use in excess of one (1) day must be submitted at least three (3) days in advance of the intended usage date. Requests for PTO use in excess of one (1) day received with less than three (3) days notice shall be approved to the extent local staffing requirements permit on a first come, first served basis. Multiple requests for the same day(s) off shall be approved in seniority order. Requests for PTO usage that are related to emergencies and other unexpected and unplanned events shall not be unreasonably denied by the Employer. Once an employee’s request has been approved, it cannot be canceled for reasons other than a major emergency. Excessive single day PTO usage in close succession and/or a regular pattern of single day PTO usage may be grounds for corrective action if such usage is not explained to the satisfaction of the Employer.”

PTO will be used for any absence of a quarter hour or more, except that the nurse may choose to use or not to use PTO for time off: 1. When a nurse is on a mandatory day off; or 2. For military leaves of absence under federal leave laws. 3. Through January 4, 2020, PTO may be used in addition to receiving workers' compensation benefits if EIT is not available, up to a combined total of PTO, EIT (if any), and workers' compensation benefits that does not exceed two-thirds (2/3) of the nurse's straight-time pay for the missed hours. 4. Effective January 5, 2020, to supplement short-term disability and paid parental leave pay to 100 percent of base pay as long as PTO is available. 5. PTO Use. A. An employee may utilize accrued PTO for sick time (including for illness of an employee’s child, parent, spouse or domestic partner) provided the employee notifies the Employer at least two (2) hours prior to the start of the employee’s shift. Requests for PTO for reasons other than sick time must be submitted at least seventy two (72) hours in advance of the intended usage date(s) and shall be approved to the extent local staffing requirements permit on a first come, first served basis. A minimum of five (5) paramedics, five (5) EMTs and two (2) Supply Techs will be approved for prescheduled PTO on any given day, provided that there is 24-hour per day coverage for Supply Techs. Additional employees above the minimum may be approved for prescheduled PTO and vacation on the same day, at the Employer’s discretion, provided adequate personnel are available to accommodate the necessary workload and the employee has sufficient accrued PTO to cover the requested time off. An employee who is denied PTO may still take the time off provided the employee obtains his/her own appropriate shift coverage in accordance with Section 10.14 and notifies the Scheduling Department at least two (2) hours prior to the shift that he/she wishes to be paid PTO for the shift. Requests for PTO usage that are related to emergencies and other legitimate unexpected and unplanned events shall not be unreasonably denied by the Employer. Once an employee’s request has been approved, it cannot be canceled for reasons other than a Government declared emergency. For call-outs on a shift immediately preceding or following scheduled PTO or a holiday, the Employer shall have the right to require verification, immediately at the start of the employee’s next shift, to establish the unforeseen nature of the circumstances necessitating the absence. Failure to provide verification to the Employer’s reasonable satisfaction may result in discipline. B. Excessive utilization of unapproved, short notice PTO (i.e., less than seventy two (72) hours) may result in disciplinary action in accordance with the employer’s attendance policy, notwithstanding the presence or absence of accrued PTO.
