ARTICLE HEALTH WELFARE. The Employer agrees to pay a percentage (as indicated below) of the present monthly billed premium of the various insurance plans set out below with the employee paying the balance through payroll deduction. Eligibility and entitlement under any of the following plans shall be subject to the terms and conditions of the plan and the requirements of the carrier as administered by the carrier. The Employer's only obligation under this provision is to pay the appropriate premiums under the various insurance plans.

ARTICLE HEALTH WELFARE. The Company agrees to provide its employees with a Health and Welfare package providing the following benefits: Extended Health Plan as set out in Appendix A Group Life Insurance Plan based on forty thousand dollars ($40,000.00) coverage per employee. This plan to include accidental Death and Dismemberment. Life Insurance benefits for laid off employees, will be paid for the duration of the lay-off up to a maximum of one year. The Company will maintain a group sickness and accident plan for all its regular employees. This plan will provide Sickness and Accident Indemnity benefits effective January of or maximum, whichever is greater, per week for a maximum of twenty-six (26) weeks of absence. Such benefits to commence on the first day of absence due to hospitalization, or non-compensable accident and on the fourth day of absence due to sickness. Initial claim eligibility under the groups Sickness and Accident Plan will be determined upon receipt of properly documented medical evidence as submitted by the employee. Prolonged absences covered by the Plan for which an employee seeks continuing benefits must be supported on request by periodic statements the attending physician. The Company will maintain a Long Term Disability Plan to provide benefits of sixty percent of the employee's base hourly rate on a monthly basis. This benefit to be effective when Sickness and Accident benefits have run out after twenty-six (26) weeks and will be payable to age or death, whichever occurs first. The Company will continue ExtendedHealth benefitsfor laid-off employees with a minimum of six (6) years' service for up to four (4) months with the first month being the month of layoff. Coverage for employees with less than six (6) years' service will end on the last day of the month in which they are placed on layoff. In both cases, reinstatement will be the first (1 day of the next month after recall. ARTICLE HEALTH WELFARE. Section The Employer agrees to pay for current non-probationary employees as of October I , per month, plus tax,for the first year of this Agreement, per month, plus tax for the second year, per month plus taxes in the third year and per month,plus tax in the fourth year, providing a health card and vision care are established for all non-probationary employees, to the TEAMSTERS (LOCAL 879) HEALTH AND WELFARE TRUST (the “Teamster Health Plan”). It is understood that funding for all such employees be made by the Company no later than October and that coverage under the Teamster Health Plan will begin for all such employees on October For all employees hired on or before October who are on probationary status on October the Company agrees to provide the same level of funding as follows: funding will be made by the Company on the day of the month following completion of probationary status and coverage under the Teamster Plan will begin for such employees on the first day of the month following the start of funding by the Company. Employer agrees to contribute towards the billed premiums from the Benefit Carrier in the indicated amounts for the following benefits for each full-time employee in the active employ of the Association who has completed probationary period, provided the balance of such monthly premiums are paid by the employee through monthly payroll deductions.
