INSURANCE, HEALTH AND WELFARE. 31.1 Employees covered by this Agreement shall be subject to the full provisions, benefits, and limitations of the University of Minnesota health insurance package provided by the Employer for the majority of employees in other non-academic bargaining units. 31.2 Employees covered by this Agreement shall be subject to the full provisions, benefits, and limitations of the University of Minnesota dental insurance package provided by the Employer for the majority of employees in other non-academic bargaining units. 31.3 Employees covered by this Agreement shall be subject to the full provisions, benefits, and limitations of the University of Minnesota life insurance package provided by the Employer for the majority of employees in other non-academic bargaining units. 31.4 Employees covered by this Agreement hired before January 1, 2010 shall be eligible for the following Employer paid Group Term Life Insurance coverage: $10,000 death coverage from any cause while employed by the Employer.

INSURANCE, HEALTH AND WELFARE. A. The Township shall continue to maintain and provide all insurance through the Employee Health Plan that is in full force and effect at the present time, except that such benefits may be increased by the provisions hereinafter set forth. Nothing in the preceding sentence shall be construed as precluding the Township from changing insurance carriers, so long as substantially equivalent coverage is maintained. B. The Township shall provide life insurance in the amount of twenty thousand dollars ($20,000). C. 1. The Township shall provide the Employee Health Plan at full cost to the Township, except that: a. All employees shall continue to pay current co-pays or coinsurance as required by each plan for all insurance coverages. b. In addition, all employees hired on or after January 1, 2006 shall also pay five percent (5%) of the cost of each prescription.

The City shall provide a comprehensive health benefit program including hospitalization, medical treatment, major medical coverage, surgical fees and office visits as follows: 80% of the first $3,000, and 100% thereafter in 2005. 80% of the first $4,000, and 100% thereafter, effective 1/1/2006. Annual dental maximum is $1,500, effective 1/12006, paid in accordance with the Summary of Benefits contained in the Plan Document. Orthodontics coverage is a maximum lifetime benefit of $3,000, effective 1/1/2006, paid in accordance with the Summary of Benefits contained in the Plan Document. 1. Mandatory pre-admission notification will be part of the comprehensive health benefit program. Lack of proper notification will reduce the level of reimbursement for health care expenses by 30%. a. Mandatory Outpatient Procedure notification will be part of the program. Single procedures that cost more than $500 and multiple procedures for a single medical problem or continuing care, which collectively cost more than $1,000 require notification. 2. For all mental care and self-inflicted injuries (in-patient, out-patient and out-of-hospital), there is a $15,000 Benefit Period maximum and a $30,000 Lifetime maximum. B. The City shall provide a vision care program for the member and his/her family to include prescription eyeglasses and/or contact lens. Coverage shall be 30% co- pay for the member and his/her family with a maximum yearly benefit of $500. C. The City shall provide, at its cost, a prescription drug plan for the individual member and his/her family. The co-pay for active members and their families is ineligible for reimbursement through the major medical part of the health plan. Prescription co-pays are as follows: Generic No co-pay Formulary Brand Name $10.00 Non-Formulary Brand Name $20.00 If, during the term of this contract, the drug formulary substantially changes, the City (represented by the Personnel Director) and the IAEP (represented by an IAEP designee) will meet to negotiate the substantial changes. D. The City shall provide yearly pediatric well care visits to include immunizations for children up to 12 years of age with a maximum yearly benefit of $400 per child, effective 1/1/2006. E. The Borough shall continue to maintain and provide all insurance coverage that is in force and in effect at the present time and shall further provide and maintain personal injury and property damage coverage, false arrest insurance and liability insurance coverage for libel, slander, defamation or violation of right of privacy, denial of due process, wrongful entry or eviction or other invasion of right of privacy, occupancy or false arrest, detention or imprisonment or malicious prosecution and assault and battery with limits up to Fifteen Million Dollars ($15,000,000) and a Seventy Million Dollar ($70,000,000) Umbrella Policy or such normal limits as provided all similarly situated municipalities in the event the Borough is a member of a joint insurance fund. Section 2. The Borough shall supply to all officers all necessary legal advice and counsel, which counsel is acceptable to the officer and the Borough, and such representation shall be provided at the Borough’s expense for any and all legal issues and problems as well as lawsuits and causes of action related to police work. Further, such legal advice and counsel shall be provided for the defense of and protection of any and all claims for personal injury, death or property damage arising out of and in the course of the officer’s employment and duties as a law enforcement officer relating to police work. In the event that it is determined that the officer acted outside the boundaries of his employment and duties as a law enforcement officer, the Borough shall have the right to seek reimbursement for all legal expenses incurred in defense of such actions. The Borough shall pay and satisfy all judgments against the officers for any such claims set forth in this Article provided the Borough had timely notice of such action and it is determined that the officer did not act outside the boundaries of his employment and duties as a law enforcement officer. Section 3. Each officer shall be furnished, at the Borough’s expense, coverage under the New Jersey State Health Benefit Program, including major medical coverage, or the complete equivalent. Coverage shall include the employee, spouse and dependents. Section 4. The officer shall be furnished dental coverage under the Xxxxxxxxx, Inc. INSURANCE, HEALTH AND WELFARE. A. The CITY shall provide a comprehensive health benefit program including hospitalization, medical treatment, major medical coverage, surgical fees and all of the benefits which are currently included in the health benefit program, at the date of this Agreement, for the employee and his/her family to commence ninety (90) days after the commencement of current active employment. Employees shall only be permitted to enroll in the type of coverage for which the employee is eligible. B. The CITY shall provide a co-pay prescription plan for the individual and his/her family. Effective January 1, 2006, the Co-payment for the Prescription Plan shall become $ 5.00 for mail in prescriptions, $ 10.00 for generic drugs and $ 20.00 for brand name drugs. C. The CITY, at its sole discretion, retains the right to select and change insurance carriers during the term of this Agreement, provided coverages are substantially similar. Disagreements regarding coverage changes can go to the grievance process and to arbitration. The CITY will notify the Union at least thirty (30) days before any change is to happen. Selection of the carrier or carriers is a managerial prerogative not subject to the terms of this collective bargaining agreement. D. Employees may choose to execute a waiver of Health Insurance program coverage and receive reimbursement per existing CITY policy. The CITY reserves the right to discontinue this policy of waiver reimbursement at any time, with 30 days notice prior to an open enrollment period. The current policy provides as follows: 1. Employees shall be permitted to opt out of the medical insurance coverage provided by the City as long as the employee furnishes the City with documentation to establish that the employee has health insurance coverage from another source. 2. Employees who elect to opt out of the coverage provided by the City shall be paid the sum of $2,000.00 for each full year that the employee declines coverage. The amount to be paid to employees who opt out for part of a year and receive City provided health insurance for part of that same year shall be pro-rated. Said payment shall be made by the City to the employee during the first pay period in December of the year in which the employee opted out of coverage. E. D. The Borough shall provide for false arrest insurance coverage in amounts of Three Hundred Thousand ($300,000.00) Dollars per individual and Five Hundred Thousand ($500,000.00) Dollars per incident protecting the Police Officer.
