Leave Procedure. All leaves as delineated in Sections 2 through 16 below are to be requested from the Employer in writing as far in advance as possible, stating all pertinent details and the amount of time requested. A written reply to grant or deny the request shall be given by Human Resources within thirty days except as otherwise provided in this Article.

Leave Procedure. The GHTA Executive Board shall manage the Catastrophic Leave Procedure (“CLP”). The CLP shall be implemented for the express purpose of providing additional sick leave days to those bargaining unit members who have suffered from a catastrophic illness or serious accident that prohibits them from returning to work and performing their normal job responsibilities. This shall also include a catastrophic illness of a spouse or child, when the spouse or child is a permanent resident of the employee’s home and when the sick leave beneficiary is a primary caretaker. a. Upon receipt of a request for catastrophic leave from a bargaining unit member, the GHTA Executive Board may approach bargaining unit members to secure donated days (using the Catastrophic Leave Donation Form) to be used to satisfy a catastrophic leave request approved under this section. The GHTA Executive Board must provide the Treasurer the original Catastrophic Leave Donation Forms so they may be processed upon approval of a catastrophic leave request. If more than one (1) person is donating days, the GHTA Executive Board will provide a list in writing to the Treasurer of the order in which sick days will be donated. b. The GHTA Executive Board may award a maximum of fifteen (15) days per request to a member who submits his/her initial request and if he/she meets the qualifications for eligibility. The GHTA Executive Board shall notify the board Treasurer in writing, the number of days a member is to be awarded. The member will only be entitled to those days which are essential. If days allotted are greater than the need, the GHTA Executive Board shall identify to the Treasurer in writing the donor member(s) to whom the unused donated days shall be returned. c. If it becomes apparent that he/she will not be able to return to work at the expiration of the first fifteen (15)-day period awarded, the member may reapply for an additional fifteen (15) days, not to exceed the maximum allotment of thirty

The county office maintains a catastrophic leave program to permit employees of the county office to donate eligible leave credits to an employee when that employee or a member of his or her family suffers from a catastrophic illness or injury. For the purposes of this section the following terms are defined as follows:

Each request shall be judged by the Sheriff on its merits and on the basis of the guidelines provided in this section. No leaves of absence, with or without pay, shall be granted unless a request is submitted by the deputy in writing to the Sheriff and/or the Board of Commissioners when necessary. Approval of leaves shall be obtained prior to the beginning of the leave periods.

The return date and time will be established and acknowledged in writing before leave commences, whenever possible. If circumstances occur when a return date and time has not been established (such as an unexpected death in the family), the Sheriff will send written notice of the scheduled return date and time to the last provided address of the Deputy via certified mail. If a change of return date and/or time is desired, the Deputy is responsible to obtain approval for the change prior to the established return date and time. If a Deputy does not return to duty on the established return date and time, it shall be considered a voluntary abandonment of the Deputy’s employment. The employment will be terminated and the Deputy will not be considered to have left employment in good standing. Should unforeseen circumstances exist that result in the Deputy being late for the assigned return time on the scheduled return date, it shall be treated as other circumstances of tardiness. Nothing in this section prohibits use of leaves as otherwise established such as sick leave.
Leave Procedure. A. An A&S staff member whose certificate status is current will be considered as if actively employed by the Board during paid leave and will be placed on the salary schedule at the level the employee would have achieved if the A&S staff member had not been absent. B. All benefits to which an A&S staff member was entitled at the time the leave of absence commenced, including unused accumulated sick leave and credits toward sabbatical eligibility, will be restored to the A&S staff member upon return. The A&S staff member will be assigned to a substantially equivalent position, if available, and for which the A&S staff member holds a valid certificate. C. The A&S staff member shall have the option of continuing as a participant in the existing insurance benefit programs at the expense of the A&S staff member while taking leave under any provision of this Article. D. All requests for extended leaves of absence, extensions, or renewals of such leaves, will be in writing and the Board will make a written response to all such requests. A&S staff members on extended leave must give notice of intention to return to work by April 1. E. Unless mutually agreed upon, employees taking leave under any provisions of this Article will return commencing July 1 of that year.
Leave Procedure. Upon application, leaves of absence may be granted employees at the convenience and discretion of the Company for limited periods not to exceed one year. Any extension beyond one year will be by agreement between the Company and Union. Employees elected or selected to perform Union duties or employees elected to public office in the City of Elkhart, Elkhart County, in the State of Indiana, or in the United States Legislature shall be granted leaves of absence with accumulated seniority until such service shall end. An employee who is on a personal leave of absence for more than 5 consecutive workdays shall pay $10.00 for family coverage, or $3.00 for single coverage for each working day that exceeds 5 days.
Leave Procedure. 8.1.1 Employees who find it necessary to be absent from duty shall notify their immediate supervisor as far in advance of the anticipated absence as possible. Employees must verify any absence and complete absence reports which may be required by the District. 8.1.2 An employee shall notify the Dispatch Supervisor or Designee by 2:00 p.m. of the working day preceding the day he/she intends to return to the job. If a driver is unable to call in by 2:00 p.m., but is able to return to work and calls in by 5:00 a.m. the next day, the driver is guaranteed his/her bid hours, but not the bid schedule. If the driver fails to call in by 5:00 a.m. and desires to work, there is no guarantee of hours or schedule. 8.1.3 Employees who request leaves of absence of other than an emergency nature shall notify the immediate supervisor at least 10 days prior to the commencement of their leave except as otherwise stated herein. Leaves of absence, when granted, shall also specify when the employee is to return from leave. 8.1.4 Employees who do not request leave in accordance with the above notification procedures shall lose one day's pay for each day of unapproved absence. 8.1.5 The District may grant or deny a request for leave and shall determine whether the requested leave will be paid or unpaid in accordance with this agreement and state and federal law. 8.1.6 Time lost due to absence may be made up upon approval of the Superintendent or designee. Such made up time does not qualify as overtime for purposes of salary payment. 8.1.7 An employee will be discharged if he/she is absent in excess of three work days without reporting to his/her immediate supervisor unless, within a reasonable time thereafter, he/she shows good cause why it was impossible for him/her to report within such three day period.
Leave Procedure a. Regular employees returning from approved leave shall be reinstated with length of service rights accumulated as of the starting date of the leave and shall not be required to serve the initial probationary period. b. Employees on probationary status returning from approved leave shall be reinstated to probationary status. The probationary period shall be extended by the number of leave days taken, where any combination of paid and unpaid leave days, other than professional development leave, exceeds thirty (30) days in a single contract year. c. Employees shall be informed in writing prior to signing their first contract whether the position they are taking is vacant due to a leave, for which the person on leave has a guaranteed right of return, and the expected ending date of the leave. Change of an employee’s status from filling a guaranteed return leave vacancy to filling an open position shall be noted in writing.
