MEDICAL/DENTAL/VISION INSURANCE. The benefits within the Medical/Dental/Vision plan shall be those described in the current Comprehensive Group Benefit Plan except as modified by changes accepted by both parties. See Section G 3, INSURANCE COMMITTEE, in this Contractual Agreement. The Board agrees to modify the Comprehensive Group Benefits Plan booklet to reflect the changes. Insurance coverage shall extend through summer to the first day of Fall Semester for all Faculty Members and their dependents (to include domestic partners) covered by this Contractual Agreement during the preceding Spring Semester. A Medical/Dental/Vision insurance plan will be provided for Faculty and their eligible dependents with a Faculty Member contribution of twenty percent (20%) of the actual premiums for the plans and options chosen by the Faculty Member (effective upon ratification of this Agreement). Prior to the beginning of each insurance plan year, the Board will make available to the Faculty Association the premium calculations and supporting documentation used in determining member contributions in each respective year. The PPO Premier Plan will only be available to unit members who have elected this medical plan prior to January 1, 2013. Should the insurance premiums for any of the health insurance plans offered by the College increase by more than ten percent (10%) over the prior year, the Insurance Advisory Committee will meet to discuss potential plan design change recommendations to reduce the premium. If the Insurance Advisory Committee chooses to recommend changes, they must be approved by a majority vote of the Insurance Advisory Committee which will then be forwarded to the Faculty Senate for review. If the Faculty Senate does not agree with recommended changes, they may offer an alternative recommendation within ten

The employer will pay the actual premium cost for medical, vision and dental insurance coverage for each participating employee, subject to an employee monthly premium cost share on a tiered rate basis, based on the employee’s medical plan selection according to the following: a. For July 1, 2014 through December 31, 2015 of the contract, the premium contributions shall be consistent with the premium share that was in place with the Union as of January 1, 2014, as part of the medical plan. b. If the parties are unable to agree on a plan for which the increase in the total plan premium does not exceed ten percent (10%), any increase over ten percent (10%) will be split equally between the Employer and each employee.
MEDICAL/DENTAL/VISION INSURANCE. During the Term, the Board will pay, as applicable, the full premium costs for medical, dental and vision insurance coverage for Superintendent and her eligible dependents under any plan approved for other professional employees of the Board, as such plans may be amended, supplemented, updated, replaced or terminated from time to time.

The District will pay $1485 monthly during the first year of the contract, $1520 in the second year and $1555 in the third year of the contract towards Medical, Dental and Vision Insurance premiums. In addition, members will each receive $35 a month for each month retro to September (October Premiums) 2022 for their insurance increase missed prior to the 2022‐2023 agreement being put in place. This will be paid in a lump sum added to the April payroll check.

MEDICAL/DENTAL/VISION INSURANCE. The City agrees to offer two Health plans from which employees may choose; The Kaiser Plan or the Regence Blue Cross High Deductible Health Plan (with a Health Savings Account (HSA)). Kaiser participants will not be eligible for the HSA account. The City’s contribution for eligible employees and their eligible dependents (as defined by the Plan Administrator) shall be 85% of the monthly premiums for City-offered health insurance (medical, dental, and vision), plus the Health Savings Account contribution for Regence Blue Cross participants only as outlined below. Employees are eligible to enroll in the City-offered health insurance if the employee is regularly scheduled to work 20 or more hours per week. For part-time employees regularly scheduled to work 20 or more hours per week, the City’s contribution to the HSA and the medical plan premiums shall be prorated based on the budgeted FTE of the position. Funding for all employees enrolled in the Regence Blue Cross High Deductible Plan to the employee's HSA account will be 100% of the deductible. Employees eligible to receive City contributions to their HSA account, will receive payments from the City on a bimonthly basis on schedule with the employee's pay period. The amount of the bimonthly contribution will equal the deductible total, based on the employee's health plan level, divided by twenty-four (24) pay periods over the year. As with the employee health coverage premium, there will be two (2) pay periods annually for which no contributions will be made. All employees regularly scheduled to work 20 or more hours per week are required to participate in the health insurance program unless the employee can prove they have attained medical insurance coverage via an alternative means. In which case, employees may opt out of the Plan and receive a $50.00/month incentive from the City. In order for the City to offer the opt out, the following must apply: 1. The employee and dependents shall be enrolled in another employer’s group health plan (e.g. a spouse’s employer group plan) that provides minimum essential health coverage as required by the Affordable Care Act, and the employee shall provide documentation of such enrollment upon each annual opt-out election and upon City request. 2. Any savings from the 2nd year (2012-13) of the HRA pool will be determined after the 90 run-off period for the plan, Jan 1, 2014.

MEDICAL/DENTAL/VISION INSURANCE. A. Medical Insurance The Agency shall pay medical insurance premiums for employees covered herein as set forth in the “Umbrella Agreement Setting the Terms and Conditions of Medical Insurance Coverage for All Santa Xxxxxx Rent Control Board Employees.” This MOU umbrella agreement is between the EAC, Hearing Examiner Representational Organization, Society for Union Employment, and Individual Unaffiliated Employees and the Santa Xxxxxx Rent Control Board. If negotiations begin for a new umbrella agreement to supersede or succeed the current Medical Insurance Umbrella Agreement between the City and the Coalition of Santa Monica City Employees, the Agency and SUE agree to meet and confer to determine how medical insurance premiums for employees covered herein will be paid. The parties will consider but not be bound by any changes agreed to between the City and the Coalition of Santa Monica City Employees. In the event that implementation of federal health insurance reform affects how public employer provided health insurance benefits are delivered and/or paid for, the Agency agrees to meet and confer with XXX. B. Dental Insurance Dental insurance coverage shall be provided at no cost to employees and their eligible dependents provided that employees covered herein participate in the City-offered dental insurance programs. C. Vision Insurance The Agency agrees to provide vision care insurance, at no cost, to employees covered hereunder. The Agency retains the right to select the provider and to set the levels of coverage for said vision care insurance plan. The Agency also retains the right to change the provider of said vision insurance plan and/or the level of benefits provided under that plan without meeting and conferring.

Retirement benefits are based on years of service and wages earned. Employees contribute 4.5% of earnings to the pension. The amount of paid time off is dependent upon the employee's position and years of service. Paid time off includes vacation, sick leave, holidays, and personal days.
