Medical Hospitalization Insurance. The District will make a direct contribution to Employee’s CalPERS medical premium equal to the minimum employer contribution under the Public Employees’ Medical and Hospital Care Act (PEMHCA) together with a contribution to the District’s cafeteria plan, to contribute up to the Kaiser Health Plan full family cost toward the Employee's medical/hospitalization insurance during the term of this agreement. If the Employee fails to make an election of health insurance benefits and/or provides proof of qualified enrollment in other group medical insurance, the Employee is not eligible to receive the employer contribution as cash.
Medical Hospitalization Insurance. For each regularly scheduled employee (other than bus aides) working five and one half (5 1/2) hours or more per day six (6) hours per day in the Food Service Department) who makes proper application to participate in the Cafeteria Benefits Plan (Educated Choices), the employer shall self-insure or pay the premium for the following (See Section (2)(a)(4) below for Bus Aides).
Medical Hospitalization Insurance. The District agrees to make available health-related insurance for each eligible employee. Such employee may elect to purchase either single or family health- related coverage. The District contribution for district sponsored health, dental or vision (if available) insurance coverage will be $200 per month. The cost of the premium not contributed by the School District shall be borne by the employee and paid by payroll deduction. Eligible employees choosing not to participate in medical-hospitalization insurance will be given a sum of $200 per month in to a district sponsored Health Reimbursement Account to spend on approved medical expenses in accordance with IRS rules, which include, but are not limited to: 1. An employee must be actively employed and enrolled in a group insurance plan to receive employer funds in an VEBA. 2. If you are not enrolled in a group insurance plan, the district will continue to provide the benefit, but you will not be able to access the funds until you meet the following conditions a. No longer employed with the school district or retirement. b. You obtain insurance under a group insurance plan. Medicare is not considered a group insurance plan.

The duties and other details related to the functioning of the committee shall be determined by members of the committee with the assistance and coordination from the Assistant City Manager/Human Resources Administrator and/or outside professional insurance personnel with experience is assisting other public sector city-wide insurance committees.

Medical Hospitalization Insurance. Subd. 1.
Medical Hospitalization Insurance. ‌ §1. The Employer shall pay for Full-Time Employees the premium cost of hospitalization and medical insurance coverage as provided by an insurer or insurers selected by the Employer. §2. Each Full-Time Employee shall have the privilege of electing to have family dependent coverage for his/her spouse and children under the medical and hospitalization insurance provisions mentioned in Paragraph 1 immediately above as the same is available through said insurer or insurers. Effective upon ratification of this agreement new Employees spouses and Employee spouses who are not covered by the employer’s medical insurance coverage are only eligible for coverage if the spouse is not eligible for coverage under his or her employer’s group medical plan. In addition to the Employer’s obligation to pay the premium cost of the coverage attributable to the individual Full-Time Employee under Paragraph 1 immediately above, the Employer shall pay the premium cost attributable to such family- dependent coverage subject to the Full-Time Employee contributions as set forth herein. A. The School District shall offer a Health Reimbursement Account (HRA) Preferred Provider Organization (PPO) Hospitalization and Medical insurance program and provide a pharmacy benefit as described below by an insurer or insurers selected by the employer. The PPO with an HRA effective on January 1, 2021, shall include an in- network deductible of $1,000/$2,000. The employee shall only be responsible for the first $500/$1,000 of the deductible. After that responsibility has been met, the District shall pay the remaining deductible expense via the HRA. This plan will only be offered for plan years 2021 and 2022 only. B. Effective January 1, 2021 the School District shall offer a Health Saving Account (HSA) –Qualified High Deductible Health Plan (QHDHP) Hospitalization and Medical insurance program and provide a pharmacy benefit as described below by an insurer or insurers selected by the employer. C. Effective January 1, 2023 this will be the only plan offered by the District. The QHDHP HSA shall include an in-network deductible of $2,000/$4,000. D. The District will fund this plan in the amount per the following: - Plan year 2021 of $1,500/$3,000. Expenses above that amount will be borne by the employee. Employer funding will be paid in full in January 2021. - Plan year 2022 of $1,500/$3,000. Expenses above that amount will be borne by the employee.
