Medical Hospitalization Insurance. Employees who are contracted to teach for the entire school year will have insurance coverage through August 31.
Medical Hospitalization Insurance. The District will make a direct contribution to Employee’s CalPERS medical premium equal to the minimum employer contribution under the Public Employees’ Medical and Hospital Care Act (PEMHCA) together with a contribution to the District’s cafeteria plan, to contribute up to the Kaiser Health Plan full family cost toward the Employee's medical/hospitalization insurance during the term of this agreement. If the Employee fails to make an election of health insurance benefits and/or provides proof of qualified enrollment in other group medical insurance, the Employee is not eligible to receive the employer contribution as cash.
Medical Hospitalization Insurance. 31.1: The Employer shall provide full family BC/BS Community Blue PPO Option 4;
31.2: At time of open enrollment and selection of benefits through the County Flex Plan, employees who provide proof of other qualified group medical/hospitalization insurance may opt out of the medical/hospitalization and dental insurance coverage and receive
Medical Hospitalization Insurance. The District agrees to make available health-related insurance for each eligible employee. Such employee may elect to purchase either single or family health- related coverage. The District contribution for district sponsored health, dental or vision (if available) insurance coverage will be $200 per month. The cost of the premium not contributed by the School District shall be borne by the employee and paid by payroll deduction. Eligible employees choosing not to participate in medical-hospitalization insurance will be given a sum of $200 per month in to a district sponsored Health Reimbursement Account to spend on approved medical expenses in accordance with IRS rules, which include, but are not limited to:
1. An employee must be actively employed and enrolled in a group insurance plan to receive employer funds in an VEBA.
2. If you are not enrolled in a group insurance plan, the district will continue to provide the benefit, but you will not be able to access the funds until you meet the following conditions
a. No longer employed with the school district or retirement.
b. You obtain insurance under a group insurance plan. Medicare is not considered a group insurance plan.
Medical Hospitalization Insurance. For each regularly scheduled employee (other than bus aides) working five and one half (5 1/2) hours or more per day (seven (7) hours per day in the Food Service Department) who makes proper application to participate in the Cafeteria Benefits Plan (Educated Choices), the employer shall pay the premium for the following (See Article 18 (C)(2)(d) for Bus Aides).
(a) Eligibility Requirements and Benefits from January 1, 2009 through October 31, 2009 for Employees Hired Prior to March 25, 2009
(b) Preferred Provider Organization (PPO)
Medical Hospitalization Insurance. Subd. 1.
Medical Hospitalization Insurance. Either party may request a contract re-opener on health insurance only prior to the expiration of this contract on December 31, 2002 by providing thirty (30) calendar days written notice to the other party.
Medical Hospitalization Insurance. Section 26.1 Effective January 1, 2021 the Employer will provide on behalf of each full-time employee and his/her family, the medical and prescription, dental and vision coverage, or comparable coverage, as contained in Appendix I (City of Oberlin Medical and Prescription Plans), Appendix II (City of Oberlin Dental Plan) and Appendix III (City of Oberlin Vision Plan). For Appendix I, employees will be permitted to select between “Plan A,” “Plan B,” or “Plan C” (See Appendices I, II, and III).
Section 26.2 Effective January 1, 2021 through December 31, 2021 the employee’s monthly contribution for each plan for family and single coverage will be as follows: Section 26.3 A city-wide joint medical/hospitalization committee comprised of two (2) representatives from the OPBA (Patrol/Sergeants Units combined); four (4) representatives from the IBEW (one (1) each from the General Maintenance Division (GMD)/Water Division/Waste Water Division/Electric Department) and four (4) non-bargaining representatives from other city departments shall be formed. The duties and other details related to the functioning of the committee shall be determined by members of the committee with the assistance and coordination from the Assistant City Manager/Human Resources Administrator and/or outside professional insurance personnel with experience is assisting other public sector city-wide insurance committees.
Medical Hospitalization Insurance. (a) The COUNTY will provide the following options for medical/hospitalization coverage:
(1) Blue Cross Blue Shield of Michigan (BCBSM) Preferred Provider Organization (PPO) with dental, vision and orthodontic riders
(2) Blue Cross Blue Shield of Michigan (BCBSM) Comprehensive Major Medical (CMM 100) The Employer may use or substitute other health insurance companies which provide comparable coverage for any or all of the health insurance component coverage (medical, prescriptions, dental, vision). The Employer will notify the Union immediately of any change or proposed change in health insurance carriers or covered benefit levels.
(b) Payment in Lieu of Health Insurance Coverage. Any active unit member who is eligible, but chooses not to participate in the medical/hospitalization insurance package, who has health insurance coverage from another source and who signs a waiver from the Employer, shall be paid an annual amount of One Thousand Eight Hundred and No/100 ($1,800) Dollars. The annual payment will be paid in equal amounts over twenty-six (26) pay dates in a calendar year. An employee who elects Payment in Lieu of Health Insurance Coverage after January 1 of any year, shall be paid a pro-rata amount of the $1,800 in the same manner as described in the previous sentence with the amount calculated based on the number of full months remaining in the calendar year after the date of the election (example: employee hired June 15, will be entitled to $900 for that year effective July 1). An employee who subsequently loses medical/hospitalization coverage from another source shall have the right to obtain medical/ hospitalization coverage from the Employer as provided in this Agreement at the earliest date possible after written notice to the Personnel Director and approval by same. Said employee shall be entitled to a pro-rata payment in lieu of health insurance to the date the employee becomes covered by the Employer's medical/hospitalization plan calculated in the same manner as described above for new hires. Any employee who elects Payment in Lieu of Health Insurance Coverage may elect at the same time to be enrolled in dental and/or vision coverage. The amount the employee will receive for Payment in Lieu of Health Insurance will be determined by deducting the cost of the dental and/or vision coverage from $1,800 (example: On January 1 employee elects family dental coverage for which the annual cost is $1,200, the employee will be paid $600 cash in lieu...
Medical Hospitalization Insurance. The School District shall contribute a sum, toward the cost of the coverage for each unit member employed by the School District who qualifies for and is enrolled in the group medical-hospitalization plan, equivalent to an amount per hour worked as follows: Year Benefit Per Hour Worked 2016-2017 $1.00 2017-2018 $1.00 Employees with current benefits greater than those noted above will remain the same until the benefit level equals the current benefits.
