EXHIBIT A SCOPE OF WORK. Project No. Scope of Services - Exhibit A Work Order Administration - Exhibit A-1 EXHIBIT A ON-CALL GEOTECHNICAL ENGINEERING AND ENVIRONMENTAL SERVICES SCOPE OF SERVICES The scope of services is as-needed/on-call based services to support the City of Mountlake Terrace. The city requires specialized geotechnical engineering and environmental related services in support of capital improvement and planning projects designed by City staff, and review and approval of development-related land use and construction permits. More information on the types of capital projects which may be involved can be found in the documents listed below, which are available on the City’s website at xxx.xxxxxxxxx.xxx. • Comprehensive Water System Plan (2018) • Comprehensive Sewer System Plan (2019) • 2007 Transportation Master Plan and anticipated 2020 update • 2021-2026 Six-Year Transportation Improvement Program • The City’s Comprehensive Plan and Town Center Subarea Plan Update EIS for background on potential development proposals The successful consultant should be a licensed expert in the area of geotechnical engineering and be able to provide specialized technical assistance, peer review, small project design services, in addition to providing City project managers with related practical and sound advice during field investigation, problem identification, and design and construction phases of projects. The consultant will be required to coordinate work with City employees as needed. Typically, work shall start within two (2) weeks, and results/reports shall be provided to the City for review within six (6) weeks of receiving a City-issued Notice to Proceed for each Work Order. Urgent needs shall require work to be performed within two (2) working days. The work included in this contract may include, but is not limited to: • Structure Stabilization (Retaining walls, foundation)‌ • Flexible and rigid pavement evaluation and design • Settlement analysis and remediation • Shored or braced excavation evaluations • Foundation investigation and design‌ • Knowledge and expertise in groundwater modeling, hydrogeology, and groundwater surface water interactions • Peer review of slope stability calculations and related design submitted by developers during the permitting process‌ • Developing landslide assessment and soils evaluation reports • Assessing slope stability including possible liquefaction • Providing geotechnical services in support of CIP projects during both design and con...

EXHIBIT A SCOPE OF WORK. 1. Downspout Disconnection generally consists of identifying the downspout leading from the roof gutter to the underground conductor (standpipe) and cutting it near the ground. An elbow and downspout extension shall be attached to the downspout to direct the flow to a grassy or other pervious area on the PROPERTY. The underground conductor (standpipe) shall be capped with a rubber sealing plug, or mortar cap to prevent debris from entering the sewer system. The downspout, elbow, extension, and any added support materials shall be fastened together and secured to remain in place. All work shall be done in conformance with Chapter 225-4 of the Milwaukee Code of Ordinances.

Contractor shall use all its expert knowledge, experience, and ability to conduct a student internship and faculty externship project as part of the DOD CADENCE Grant Project 9: Internships/Externships for Dual Use Innovation to assist DoD Small Business Innovation and Research (SBIR) Phase I, II, III awardees with commercialization efforts of dual use innovations, which includes, but is not limited to, the following: Deliverables: Identify at least one faculty member to participate in externship and act as faculty coach(es) for student interns. Recruit a minimum of two students to participate as student interns with DoD companies. Promote and assist students with enrollment in the college’s Work Experience/Work Study course so that students can earn college credit for their work experience as interns. Ensure that the faculty coach/extern submits the final report documenting the results and outcomes of the project using the final report template attached as Exhibit B to this Agreement. Assist in matching student interns with a DOD SBIR Phase I, II, or III company. Recruit a minimum of two students to participate as student interns per DOD SBIR Company. (Colleges will be provided with a database of California DOD SBIR firms and outreach materials to solicit interest in the student internship and faculty externship project.) Execute Work Site Agreements and External Work Site Agreements with Calbright and the Foundation for California Community Colleges (“FCCC”), as requested. (Calbright will be entering into a Career Catalyst Program Agreement with FCCC for the FCCC to be the employer of record for the student interns. For virtual internships, the participating colleges will be considered the Work Site and the Career Catalyst Program Agreement will require Calbright to enter into a Work Site Agreement with each participating college. For on-site internships at DOD SBIR Companies, FCCC will also require a three-way External Work Site Agreement be entered into by Calbright, the participating college, and the DOD SBIR firm where the student will be working as an intern. OVERVIEW The Environmental Monitoring and Technical Services (EMTS) Division of Public Utilities Department (PUD) operates two main and several small laboratory facilities which conduct scientific analyses in support of water and wastewater operations. The variety and technical difficulty of the scientific analyses performed by EMTS laboratory facilities require the use of advanced and state of the art instruments that are operated by a highly trained staff of Chemists. These instruments require frequent preventive maintenance and as-needed repair services to ensure proper functioning of the system at all times according to standard industry practices and usage.

EXHIBIT A SCOPE OF WORK. Consultant will provide the City with labor, materials, supplies, equipment, facilities, disposal, photometric analyses, digital reports, and processing for all utility rebates and rate schedules associated with the conversion program. This program will replace mostly high pressure sodium (HPS), cobra head and shoebox style street light fixtures to Light Emitting Diode (LED) to the City. The work to be done consists of furnishing and installing new LED energy efficient lighting fixtures, removal and disposal/recycling of existing HPS lighting fixtures and maintaining existing electrical systems working during construction. No used, refurbished, reconditioned, or rebuilt parts or products shall be used, unless otherwise approved by the City in writing; all new LED street lights shall be fully operational each night unless documented. The services to be provided hereunder will be performed in two phases: The Audit and Development phase and the Project Implementation phase. These phases are described in more detail below. Consultant will be deemed to have satisfactorily completed the Scope of Work with respect to the Audit and Development phase (the “Deliverable 1 Work”) upon delivery to the City of the deliverables required under the heading Deliverable 1. With respect to the Project Implementation phase, Consultant will be deemed to have satisfactorily completed the Scope of Work with respect to the Project Implementation phase (the “Deliverable 2 Work”) upon delivery to the City of the deliverables required under the heading Deliverable 2.

EXHIBIT A SCOPE OF WORK. The Cure Violence Epidemic Control (Health) Model is a data-driven, research-based, and community- centric approach to violence prevention. Violence clusters, spreads, and transmits like a contagious disease which are the characteristics of an epidemic. By partnering with the Columbus Health Dept and taking the health approach we are re-understanding violence to reduce the inequities of marginalized communities and promote understanding of the contributing factors of violence. It has been shown that a person’s risk of adopting violent behavior is increased based on their exposure to violence. It is time to view this untreated and misdiagnosed health problem through a new lens. The Cure Violence program utilizes street and hospital level intervention techniques based on a public health perspective. The program would be implemented and managed under the Columbus Health Dept to provide accountability, develop intervention strategies, guide partnership direction, and manage funding distribution to successfully reduce violence exposure. The model has specificity that includes five total components that are required for it to be a successful Cure Violence program. Three Core Components
