EXHIBIT A SCOPE OF WORK. Project No. Scope of Services - Exhibit A Work Order Administration - Exhibit A-1 The scope of services is as-needed/on-call based services to support the City of Mountlake Terrace. The city requires specialized survey and CAD related services in support of capital improvement projects designed by City staff, and review and approval of development-related land use and construction permits. Internally designed capital projects are typically smaller in scale while larger projects will utilize a separate on-call Engineering consultant(s) and their partnered land surveying services. More information on the types of capital projects which may be involved can be found in the documents listed below, which are available on the City’s website at xxx.xxxxxxxxx.xxx. • Comprehensive Water System Plan (2018) • Comprehensive Sewer System Plan (2019) • 2007 Transportation Master Plan and anticipated 2020 update • 2021-2026 Six-Year Transportation Improvement Program • The City’s Comprehensive Plan and Town Center Subarea Plan Update EIS for background on potential development proposals The successful consultant should be a licensed expert in the area of land surveying and be able to provide surveying, preparation of base plans, legal descriptions, CAD and GIS work, and related services. The consultant will be required to coordinate work with City employees as needed. Typically, survey work shall start within two (2) weeks, and base plans shall be provided to the Engineer for review within six (6) weeks of receiving a City-issued notice to proceed for each Work Order. Surveyors and CAD technicians shall be available upon two (2) working days’ notice at the quoted hourly rates for potholing and construction phase work. Information provided by the City will include: • Electronic versions of mapping layers from the City GIS system for the project area. Although GIS based topographic information is available, it is not accurate enough for detailed design. • City-owned utility and As-Built records for the general area of the project upon request. • City of Mountlake Terrace CAD StandardsA list of benchmarks around the city, typically at monuments, based on NGVD1929 and NVD1988. Some project sites do not have monuments nearby due to loss from past construction activities. • Compiling and reviewing available information related to the project, including plats, maps, GIS information, utility plans, easement documents, deeds, and design drawings • Performing site reconnaissance...

EXHIBIT A SCOPE OF WORK. Downspout Disconnection generally consists of identifying the downspout leading from the roof gutter to the underground conductor (standpipe) and cutting it near the ground. An elbow and downspout extension shall be attached to the downspout to direct the flow to a grassy or other pervious area on the PROPERTY. The underground conductor (standpipe) shall be capped with a rubber sealing plug, or mortar cap to prevent debris from entering the sewer system. The downspout, elbow, extension, and any added support materials shall be fastened together and secured to remain in place. All work shall be done in conformance with Chapter 225-4 of the Milwaukee Code of Ordinances.

Recruit a minimum of two students to participate as student interns per DOD SBIR Company. (Colleges will be provided with a database of California DOD SBIR firms and outreach materials to solicit interest in the student internship and faculty externship project.)‌ • Execute Work Site Agreements and External Work Site Agreements with Calbright and the Foundation for California Community Colleges (“FCCC”), as requested. (Calbright will be entering into a Career Catalyst Program Agreement with FCCC for the FCCC to be the employer of record for the student interns. For virtual internships, the participating colleges will be considered the Work Site and the Career Catalyst Program Agreement will require Calbright to enter into a Work Site Agreement with each participating college. For on-site internships at DOD SBIR Companies, FCCC will also require a three-way External Work Site Agreement be entered into by Calbright, the participating college, and the DOD SBIR firm where the student will be working as an intern. YANA M2M received $80,000.00 in year one and, $180,400.00 in year two, and is receiving $180,960.00 in year three. Excerpts from YANA M2M’s scope of work response in the grant application are included in Exhibit A to describe the elements that will be funded through this grant program. The County expects this scope of work to be executed during the funding term.

The scope of work for this project involves an annual service agreement with the City of Milpitas for replacement of defective/obsolete Fire Hydrants and appurtenances according to City specification. Contractor will provide all necessary material, labor, equipment, and resources necessary to complete the Task Orders as given in this contract. Task Orders performed under this Contract may consist of, but not necessarily limited to, the following tasks: 1. Removal and replacement of 20 fire hydrants and appurtenances according to City specifications. 2. Provide new Fire Hydrant Assembly, including Xxxx Model LBI 400A Break Off Check Valve for commercial and industrial areas. 3. Removal and replacement of hydrant riser, bury, and all piping necessary to connect hydrant to the hydrant auxiliary (shutoff) valve if the existing riser, bury and piping are corroded or otherwise no longer in serviceable condition when exposed. 4. Removal and disposal of existing pavement or structures required to construct new fire hydrants. 5. Placement of asphalt paving or reinforced concrete paving over areas excavated for the hydrant installation. 6. Placement of sidewalks and driveways over areas excavated for the hydrant installation. 7. Removal and replacement of brick paved streets. 8. Restore any damaged lawns or landscaping. 9. Provide Hydromulch seeding or sodding in areas disturbed by construction. 10. Provide traffic control. 11. Provide trench safety systems.
EXHIBIT A SCOPE OF WORK. Consultant will provide the City with labor, materials, supplies, equipment, facilities, disposal, photometric analyses, digital reports, and processing for all utility rebates and rate schedules associated with the conversion program. This program will replace mostly high pressure sodium (HPS), cobra head and shoebox style street light fixtures to Light Emitting Diode (LED) to the City. The work to be done consists of furnishing and installing new LED energy efficient lighting fixtures, removal and disposal/recycling of existing HPS lighting fixtures and maintaining existing electrical systems working during construction. No used, refurbished, reconditioned, or rebuilt parts or products shall be used, unless otherwise approved by the City in writing; all new LED street lights shall be fully operational each night unless documented. The services to be provided hereunder will be performed in two phases: The Audit and Development phase and the Project Implementation phase. These phases are described in more detail below. Consultant will be deemed to have satisfactorily completed the Scope of Work with respect to the Audit and Development phase (the “Deliverable 1 Work”) upon delivery to the City of the deliverables required under the heading Deliverable 1. With respect to the Project Implementation phase, Consultant will be deemed to have satisfactorily completed the Scope of Work with respect to the Project Implementation phase (the “Deliverable 2 Work”) upon delivery to the City of the deliverables required under the heading Deliverable 2.

EXHIBIT A SCOPE OF WORK. Contractor shall perform a Step Rate Test on the Firestone SWD #1 injection well subject to the procedure outlined in EPA Permit CO12413-11960. The test shall be performed, and the data logged, in such a manner as to be compliant with EPA requirements and protocols and with expectation that the data collected will be submitted to EPA by the Authority as part of an application to increase the Maximum Allowable Injection Pressure for the well. Contractor shall remove the existing fiberglass shelter that protects the well head. The Authority will disconnect all of the interior conduits cabling, and anchors. When the work is completed, the Contractor shall reinstall the fiberglass shelter over the well head. The Authority shall reinstall the conduits, cabling, and anchors. Contractor shall provide and install down hole pressure gages at the bottom of the well that shall include instantaneous surface readout and data logging at 1 second intervals or less. Contractor shall supply frac tanks and injection fluid, injection pumps, and associated plumbing to perform the step rate test at the rates and volumes identified in the Contractors Proposal, attached as Exhibit B to this Agreement. Contractors’ equipment shall meter and continuously log flow rates and volumes of injected fluid. - In connection with the above, contractor shall ensure that the step rate test may be conducted at a rate of 40 barrels per minute with 4,500 psig of pressure. - Contractor, post step rate test and retrieval of gauges, shall acidize well with 10,000 gallons of 15% equivalent HCl concentration of non-corrosive synthetic acid. (Enviro-SYN® HCR-7000-WL-FRTM Modified Acid). Acid shall be displaced at a rate of 40 barrels per minute. Work shall be coordinated with IPT Well Services to ensure that all of date required for submittal to the EPA is collected and logged. Prior to the start of the work, Contractor, IPT, and the Authority will meet to review the work plan. Contractor shall NOT be responsible for revegetation of the work area but is to restore the surface grade to pre-construction grades if they are disturbed to eliminate ruts or low areas that would hold water.
