Formal Evaluation Procedure. 1. All certificated instructional employees must receive an evaluation in which at least eighty percent (80%) of the evaluation results are based on Professional practice. All measures included within the Professional Practice portion of the evaluation must be aligned to the Xxxxxxxxx Xxxxxxxxx Framework for Teaching Second Edition. The measures included within the professional Practice portion of the evaluation shall include a minimum of two (2) documented observations annually, with at least one (1) observation being completed by January 1 of each year. District evaluation models shall also include at least one (1) of the following as a measure to inform the professional Practice portion of all certificated instructional employee evaluations: a) Parent/ guardian input; b) student input; and/or c) Portfolios. 2. All certificated instructional employees must receive an evaluation in which at least thirty-three percent (20%) of the evaluation results are based on multiple objective measures of growth in student achievement as determined by the board of trustees and based upon research. For evaluations conducted on or after July 1, 2014, growth in student achievement as measured by Idaho’s statewide assessment for Federal accountability purposes must be included. This portion of the evaluation may be calculated using current and/or past year’s data and may use one (1) or multiple years of data.

Formal Evaluation Procedure. 7.2.1 Every probationary certificated employee shall be evaluated by the administration in writing a minimum of twice each school year. The first such evaluation shall be completed no later than December 1, and the second shall be completed no later than March 15. 7.2.2 Every permanent certificated employee shall be evaluated by the administration in writing a minimum of every other year. The final evaluation shall be filed no later than thirty calendar days prior to the end of the school year. 7.2.3 An evaluation shall be completed at least every five years for personnel with permanent status who have been employed at least 10 years with the school district, are highly qualified as defined in 20 U.S.C. Section 7801, and whose previous evaluations rated the employee as meeting or exceeding standards, if the evaluator and the certificated employee being evaluated agree. The certificated employee or the evaluator may withdraw consent at any time. Both the evaluator and the certificated employee must provide written consent annually and note the year of the evaluation cycle. 7.2.4 No later than the end of the seventh school week of the year, the evaluator and the certificated employee shall meet and mutually agree to instructional goals and objectives for the school year upon which evaluations of the employee are to be based. 7.2.5 During the course of the evaluation period, circumstances may arise which require modification of the evaluation process. Such modifications shall be accomplished by mutual written agreement between the evaluator and the evaluatee. 7.2.6 The formal written evaluation of a teacher shall be based upon a number of classroom observations of sufficient length to enable the evaluator to form a valid opinion of the teacher's work. 7.2.6.1 Lesson observation(s) for the purpose of formal evaluation, shall normally be arranged by the evaluator and the evaluatee by mutual agreement at least 48 hours in advance of the observations. Short-term observations may be carried out on an unannounced basis. 7.2.6.2 Any certificated employee who receives an unsatisfactory observation may, upon request, be entitled to a subsequent observation, conference and written evaluation, as prescribed above. Formal Evaluation Procedure. 1. There shall be a minimum of one (1) written evaluation in each of the annual contract years of employment. The first portion of which shall be completed before February 1of each year and shall include input from parents and guardians of students as a factor. A second portion shall be included for all evaluations conducted after June 30, 2012. This second portion shall comprise at least fifty (50%) of the total written evaluation and shall be based on objective measure(s) of growth in student achievement. The requirement to provide at least one (1) written evaluation does not exclude additional evaluations that may be used. (I.C. 33-513) 2. Category A teachers. Will be observed and evaluated two times per year with the formal evaluation tool. Teachers will fill out the Pre-Observation Form prior to meeting with the Principal. Principal and Teacher will meet at least one (1) day before the observation. Teacher will fill out Post-Observation Reflection Form and return it to Principal at least one (1) day before the Post- Observation meeting. After the observation is completed, Principal and Teacher will meet to go over observation and evaluation. 3. Category B teachers will be observed and evaluated a minimum of one time per year with the formal evaluation tool. Category B teachers will also complete a Professional Development Plan. The Principal will go over the Professional Development Plan with the Teacher within the first six (6) weeks of school to approve the plan. Adjustments can be made at that time. At the end of the year, the Teacher and Principal will go over the Professional Development Plan Summative Appraisal together. 4. “Grandfathered Renewable Contract” teachers. Grandfathered Renewable Contract teachers will be observed and evaluated a minimum of one time per year with the formal evaluation tool. Grandfathered Renewable Contract teachers will also complete a Professional Development Plan. The Principal will go over the Professional Development Plan within the first six (6) weeks of school to approve the plan. Adjustments can be made at that time. At the end of the year, the Teacher and Principal will go over the Professional Development Plan and Summative Appraisal together.

Formal observations will be conducted at least 30 days apart. 3. At least one (1) of the observations must be announced at least two (days) prior to the observation. c. Within ten (10) school days after each observation on which the evaluation is based, the teacher will be provided written feedback on that observation. If requested, a post-observation conference to discuss the feedback may be arranged. d. A copy of the year-end formal evaluation resulting from the observation(s) shall be given to the teacher at least ten (10) school days before the last day of school. If requested, a post-observation conference to discuss the evaluation may be arranged. e. Each teacher shall sign each evaluation upon the completion of the post-observation conference for the purpose of indicating thereon that the evaluation has been reviewed by the teacher and the evaluator. Signing the form in no way indicates agreement. f. If the teacher disagrees with any part of the written evaluation, then s/he may submit within ten

Formal observations will be conducted at least 30 days apart. 3. At least one (1) of the observations must be announced at least two (2) days prior to the observation. c. Within ten (10) school days after each observation on which the evaluation is based, the ancillary staff will be provided written feedback on that observation. If requested, a post-observation conference to discuss the feedback may be arranged. d. A copy of the year-end formal evaluation resulting from the observation(s) shall be given to the ancillary staff at least ten (10) school days before the last day of school. If requested, a post- observation conference to discuss the evaluation may be arranged. e. Each ancillary staff shall sign each evaluation upon the completion of the post-observation conference for the purpose of indicating thereon that the evaluation has been reviewed by the ancillary staff and the evaluator. If the ancillary staff disagrees with any part of the written evaluation, then s/he may submit within ten (10) school days a written rebuttal that shall be attached to the evaluation and placed in his/her personnel file.

Formal Evaluation Procedure. The annual evaluation summary consists of all evidence collected during the probationary period, such as: observation evidence, professional development evidence, and/or documentation reflecting collaboration with other faculty members, from a colleague or mentor (during the 4th – 6th years), gathered over the course of the year. Evidence may also include information gathered by the faculty member or Evaluating Administrator, such as student or parent feedback, special projects or committee work, and special awards or recognitions. The Evaluating Administrator completes the annual evaluation for every probationary faculty member.
Formal Evaluation Procedure. A formal evaluation plan shall be developed by a joint committee of the WEA and the Administration to address those changes in The School Code relating to evaluations that must be implemented during the term of this Agreement. A. Each tenured certified staff member shall be evaluated at least once every two years. Non-tenure certified staff members shall be evaluated once or more each year. All monitoring or observation of the work of each employee shall be conducted in person and any additional monitoring or input from other persons shall be with full knowledge of the certified staff member being evaluated. The evaluation sheet will be reviewed jointly by the certified staff member and the principal. Employees who have floating or multiple building locations shall be assigned a single supervisor for purpose of evaluation. E. A certified staff member may submit additional written comments to the written evaluation within 30 days of receipt of the written evaluation if he/she so desires. All written evaluations and the certified staff member’s comments are to be placed in the certified staff member’s personnel file. F. The certified staff member will receive a copy of the evaluation and all recommendations made by the administrator which are to be placed in the certified staff member’s personnel file. G. During the summative evaluation conference, the certified staff member will sign the evaluation report. In no case shall the certified staff member’s signature be construed to mean that he/she necessarily agrees with the contents of the evaluation. H. The Administration will strive to uniformly apply the procedures of evaluations throughout the District. Problem areas of uniformity may be directed to the joint committee. I. Evaluation procedures: 1. Within one month after the beginning of the fall term, the building principal shall inform each certified staff member to be evaluated that school year of the evaluation procedures, standards and instruments to be used and each certified staff member’s right of response to the principal’s evaluation. 2.
