Health & Dental Insurance. 1. The District shall pay the following premiums for individual employees’ health and dental insurance for the life of this contract:
a. For employees that elect dental insurance, the District shall provide a maximum of $10.80 per month towards the elected dental insurance in accordance with the term and schedule of benefits currently in effect.
b. The District agrees to pay 100% of the cost of the health insurance premium equivalent for employee-only coverage for the three (3) health insurance plans: Premier Plus Plan, Premier Plan, and Premier Choice Plan. Employees will continue to pay the cost of the health insurance premium for any coverage selected other than employee-only coverage. For future plan years: The parties are committed to working through the Superintendent’s Insurance and Wellness Advisory Committee to implement cost effective improvements to the group health insurance program, such as the increased usage of: • Teladoc • Wellness Programs • Extended Rx offerings • Urgent Care over Emergency Room visits, when feasible The District shall separately account for costs of group insurance program within the General Fund in a manner that allows costs and savings to be readily identified. At the end of each fiscal year, any savings in the overall cost of the insurance programs (defined as the total premium equivalent contributions for that year compared to the projected total premium contributions for the next year) shall be placed in a reserve which may only be used to fund future employee supplements, and/or other benefits for employees, prior to the end of the next plan year.
c. All new employees may elect to enroll in a District offered plan of their choice subject to conditions for enrollment in such plan.
d. Benefit levels, plan design, cost containment for the plan, premium contributions and benefit improvements will be reviewed annually by the Superintendent Insurance and Wellness Advisory Committee. The Superintendent’s Insurance and Wellness Advisory Committee will make recommendations to the Superintendent about benefit levels, plan design, cost containment for the plan and benefit improvements. Either party may demand negotiations through the Superintendent or designee to amend benefit levels, plan design, cost containment for the plan and benefit improvements, or premium contributions.
e. An employee eligible for health insurance may voluntarily decline such insurance. An employee declining medical insurance shall be required to sign...
Health & Dental Insurance. For the term of this Agreement, group medical insurance benefits, including dental benefits, will be offered to eligible bargaining unit employees (excluding retirees).
Health & Dental Insurance a.) Full-time licensed staff participating in the District's health insurance coverage shall contribute a specific amount for each contract year toward the premium for single health and dental coverage. Beginning with the 2021-2022 school year, these single contributions will increase or decrease annually at the same rate as the District’s medical/dental plan for those staff members electing single coverage only, either PPO or HMO. The Board shall pay the balance of the premium for single health and dental coverage for all full-time licensed staff, except as provided below. Licensed staff electing to enroll in an HMO program will be permitted to apply no more than the monthly Board single contribution toward such payment, with any additional amount becoming a payroll deduction. The Board shall pay 70% of the premium for family health and dental coverage for full-time licensed staff with four (4) or more years of service in the District who so elect such coverage. Part-time licensed staff covered by this Agreement shall receive the same health and dental coverage as full-time licensed staff on a prorated basis, with employee contributions also prorated.
b.) If the District receives notice of an increase in the individual health and dental premiums based on bargaining unit membership of more than five percent (5%) over the prior year's premium, the Administration will inform the Association's President(s) as soon as possible. The Association may convene the Insurance Committee consisting of a mutually agreed number of Association and Administration members to discuss options to change the District's health and dental coverage in order to reduce such premium increases. Additional representatives from employee groups will be invited to participate on the committee. The Insurance Committee shall meet at least twice annually to discuss current information concerning costs and benefits. The Association will thereafter advise the Administration in writing of any approved plan changes, and will do so within the time frame established by the District's insurance provider.
c.) If the final premium for individual health and dental coverage increases by more than five percent (5%) over the prior year premium, licensed staff electing such coverage shall pay one-half of the increased cost over five percent (5%), in addition to the base annual contribution.
Health & Dental Insurance. The District shall contribute the sum of four hundred dollars ($400.00) per month ($4800 per year) for the 2018-2019 fiscal year and four hundred dollars ($400.00) per month ($4800 per year) for the 2019-2020 fiscal year toward the premium for health insurance for each eligible registered nurse who elects to enroll in the District’s health insurance plan. A registered nurse electing more expensive coverage than the Districts contribution will pay the additional premium cost through payroll deduction. The District shall contribute the sum of sixteen dollars and sixty-six cents ($16.66) per month ($200 per year) for each eligible registered nurse who elected dental insurance for the duration of this agreement. The balance of any premium costs shall be borne by the registered nurse and paid by payroll deductions.
Health & Dental Insurance. 1. The Board shall provide health and dental insurance for eligible bargaining unit members. As to those members eligible under Paragraph 2 of this Section below, the employee will contribute (by payroll deduction) 10.5% of the monthly cost of such insurance (single, two-party, or family coverage, whichever is applicable).
2. Health and dental insurance coverage will be provided to all bargaining unit members who regularly work thirty (30) hours or more per week. For multiple positions, the number of hours required for insurance eligibility is thirty-three (33).
3. The Board shall contribute toward the monthly cost of health and dental insurance for any bargaining unit member who regularly works at least ten (10) hours per week but less than thirty (30) hours per week (33 hours per week for multiple job assignments) in accordance with the following schedule: Hours Regularly Worked Per Week Board Contribution 25-29.99 (32.99 for multiple job assignments) 60% 10-24.99 50%
4. The Board will offer employees an Employee Assistance Program, the cost of which will be paid by the Board.
Health & Dental Insurance. A program of Health Insurance, including Hospital, Surgical, Dental, and Major Medical benefits is provided for the District employee. Each BEST classified staff member shall pay a percentage of the premium cost for single, couple, or family coverage for both health including dental insurance. The percentage of premium will be based on the fully insured equivalent rate which will be shared annually with the BEST union. The following contribution rates represent the percentage of premium for each contract year: The parties agree that when a new employee is hired and elects the HMO insurance coverage plan, they will have coverage under the Blue Advantage Network. Health including Dental at 20% of the premium cost Health including Dental at 20% of the premium cost Health including Dental at 20% of the premium cost Health including Dental at 20% of the premium cost Coverage shall begin after 90 days of employment in the District. The dental plan shall pay a maximum benefit for each calendar year of two thousand ($2,000.00) dollars which amount applied separately to each insured family member. The Board contribution to insurance for the District employee shall continue for a six (6) month period even though the District’s employee’s sick leave has terminated. All affected employees have the obligation to return to the District or shall owe the District the specific amount of premiums paid.
Health & Dental Insurance. A. The Board shall pay, on behalf of each requesting certified staff member, eighty percent (80%) per month for a single or family plan of coverage for the health care plan that is in effect on the effective date of this Agreement, or a plan equivalency.
B. The Board reserves the right to change carriers, but the coverage must be equal to or better than present coverage.
C. The Board shall pay, on behalf of each requesting certified staff member, eighty percent (80%) per month for a single or family plan of coverage for the dental care plan that is in effect on the effective date of this Agreement, or a plan equivalency. All eligible employees may receive an opt-out bonus if a minimum of 20 employees elect to dis-enroll or not enroll in the health insurance plan offered by the Board. Written notice of the employee’s election to dis-enroll or not enroll in the Board’s plan must be received by the Board no later than August 31st of each year. Each year that the minimum number of employees elect to opt-out of the Board’s health insurance plan, the Board shall pay a cash incentive of $1,100 to each eligible employee who elects to dis-enroll or not enroll in the Board’s “single” health insurance plan, and $2,250 to eligible employees who are eligible for and elect to dis-enroll or not enroll in the Board’s “family” health insurance plan. Any employee who is hired on or after October 1st and who would be eligible to enroll in the health insurance plan offered by the Board shall receive a $550 bonus for “single” plan and a $1,275 bonus for “family” plan if the employee elects to not enroll in the Board’s health insurance plan. Proof of coverage in a health plan not provided by the Board must be provided in order to qualify for the aforementioned cash incentives. The cash incentive shall be paid on an annual basis by payroll check on the first payroll in December of each year to qualified employees on the Board’s payroll as of November 30. An employee who loses their health insurance coverage through a plan not provided by the Board may elect to re-join the health insurance plan offered by the Board. In such instance, any cash incentive received by the employee shall be repaid to the Board, and the Board shall be authorized to deduct from the employee’s paycheck those amounts to repay the cash incentive. The payback of the incentive shall be through a monthly deduction of one-third of the incentive for a period of three months, which in no event shall extend beyon...
Health & Dental Insurance. Administrator shall be entitled to health and dental insurance comparable to the amount of such benefits received by the certificated teaching staff employed by the District during the term of this agreement.
Health & Dental Insurance. The District will pay the Superintendent’s cost of health and dental insurance for the Superintendent, the Superintendent’s spouse, and children, through the District’s group insurance plans. (Equivalent to, but not exceeding, the “Family” plan)
Health & Dental Insurance. As a full-time employee, you will continue to be eligible for medical and dental benefits. The Company offers several plans and shares the cost of the monthly premium with you. You may choose which plans satisfy your personal and family circumstances. In addition, you have the option to purchase vision coverage and may set up a flexible spending account. The Company reserves the right to modify its benefit program at any time.
