Health/Medical Insurance. The Board’s contribution shall not exceed $550.73 per month, per employee, for those employees who “opt” in, and this shall be negotiated annually. Employer agrees to renegotiate health insurance premiums for bargaining unit members if another bargaining unit receives a Board contribution in excess of the one stated in this contract. The effective date of insurance for all new hired employees will be the 1st of the month following a 60 day wait period effective January 1, 2015. Any funding received by the District as a result of the insurance profit sharing program will be negotiated to offset health insurance costs for bargaining unit members.

Health/Medical Insurance. Teachers eligible for the severance benefit, as established by Article XV (A) (2), may elect to continue to participate in the hospital/medical- insurance program and the dental insurance program, as established by Article XV. Participation in the referenced insurance programs may continue indefinitely pursuant to Minn. Stat 471.61. The Employer shall pay up to $320 of the monthly premium cost of the single, hospital/medical insurance available to a Teacher under this Master Agreement for which the Teacher is eligible and enrolled. The Employer's contribution will continue until the participant becomes eligible for Medicare or another insurance program. A Teacher electing to participate shall pay the balance of the full, monthly premium for hospital/medical insurance and the full, monthly premium (single and family) for the dental-insurance program for which the Teacher is eligible and enrolled.
Health/Medical Insurance. Employees who are eligible for the severance inducement benefit as established by this ARTICLE may elect to continue to participate in the hospital-medical and dental insurance programs, as established by Article 18, pursuant to MN.Statute 471.61. Employees electing to participate shall pay the full monthly premium cost of coverage (single or family) for which the employee is eligible and enrolled.
Health/Medical Insurance. The Town agrees to provide hospitalization insurance which includes traditional coverage, preferred provider organization and health maintenance organization through the New Jersey State Health Benefits Program, as exists or as modified by the State Health Benefits Program (or any other equal to or better health benefit plan) including any changes in co-pays or deductibles that may be implemented by the State Health Benefits Program, for all employees and eligible dependents covered by this Agreement.
Health/Medical Insurance. 11.2.1.1 For each eligible employee and each verified dependent, the district will make a monthly contribution for medical insurance through SISC or another carrier selected by the district, effective October 1, 2004. 11.2.1.2 Except when authorized under Article 10.8.3.1 or elsewhere in this Agreement, employees working partial assignments shall receive a prorated district contribution based on the percentage of their assignment as provided in Section 11.1.4. 11.2.1.3 Health/medical insurance coverage for the employee is mandatory except that an employee who submits proof of duplicate coverage at a level equivalent to the district plan may be excused from the plan. 11.2.1.4 Cash-in-lieu: Unless otherwise prohibited by the policy carrier, the District shall provide 25 percent of the district contribution as cash-in-lieu to those employees who meet the criteria listed in 11.2.1.3 and opt out of the District’s medical insurance plan. The contribution amount shall be based on single coverage for the lowest available employee plan. 11.2.1.5 The district is not obligated to pay any increase in premium cost after September 30, 2004. Any increase in cost shall be borne by the employee through automatic payroll deduction. Any such deduction shall be the difference between the new premium and the district contribution listed below. Prior to implementing payroll deductions, the district will meet with employee groups to consider possible alternatives, such as a different carrier, revisions to coverage, or changes in deductibles. Effective October 1, 2014, the district will pay $340.00 per month for single coverage, $673.00 per month for two-party coverage, and $953.00 per month for family coverage.

Health/Medical Insurance. 12.1.1 The District shall provide coverage for health/medical insurance for members of the unit through plans recommended by the unit representative on the District Insurance Review Committee and adopted by the District. The SEIU unit may appoint a unit member as their representative to this committee. 12.1.2 The District shall make a monthly contribution for the employee and dependent(s) medical insurance coverage up to the lowest premium rate established by any of the District's carriers for the plans that are available to SEIU members. Should the premium rate structure for District health and/or dental plans be changed to a tiered structure, the District shall meet and confer with Union representatives regarding the effects of such a change. 12.1.3 The established District contribution levels for full-time unit members as described in Appendices A and B for health/medical coverage shall be applied towards the premium cost of the selected plan. 12.1.4 Unit members working less than full time will receive a proportionate share of the District contribution as the number of hours worked by the employee in one month bears to the number of hours worked by a full-time employee, provided the employee agrees in writing to have the remaining amount deducted from his/her monthly warrant. 12.1.5 The District contribution for medical premiums will be made to the District’s carriers. The carriers currently providing medical coverage to SEIU members in 2005-06 are: Health Net Plan Kaiser Health Plan PacifiCare Health Plan 12.1.6 The District will pay the above-stated premium for medical insurance coverage for the employee as long as he/she remains in the paid status. Premiums for nine-, ten-, and eleven-month employees will be paid by the District during regular non- pay months as prescribed in Section 12.1.

Health/Medical Insurance. The health Major Medical Insurance as stated in this contract is the United Healthcare of Ohio. The Board may make additional plan(s) available. The parties agree that the premium health care plan offered by the District will be eliminated on September 30, 2017. Hospitalization, dental and vision insurance will be available to all regular contract unit members who are employed for half time or more. The cost of hospitalization shall be eighty-five percent (85%) paid by the Board and fifteen percent (15%) paid by the employee. The cost of dental and vision insurance shall be shared between the employee and the Board with the Board paying eighty percent (80%) of the premium and the employee paying twenty percent (20%). Health/Medical Insurance. 1. Effective January 1, 2017, the Town of Xxxxxxx shall select a Carrier to provide Health/Medical Insurance coverage for its employees. The Town shall pay the cost of the monthly premium for single, two (2) person or family coverage, except that each employee will contribute the equivalent of 25% of the current rate for the plan selected from Section A below, inclusive of any premium adjustments that may change from year to year. Employees who would otherwise be eligible for Town Health/Medical Coverage, who elect Health Insurance Coverage under their spouse's plan, or another comparable insurance plan, the cost of which are paid for by an employer other than the Town, will be eligible for compensation in lieu of the Town's Health Insurance Plan. Effective April 1, 2017 eligible employees will be compensated $2,500 annually for waiver of the single plan benefit, $8,000 annually for waiver of the two-person plan benefit, or $10,000 for waiver of the family plan benefit. Applicable stipends shall be prorated and paid monthly as long as the employee maintains the waiver. a) To be eligible for this benefit employees must meet the following criteria: I. Have and show proof of coverage in a comparable Plan. II. Initially, attend informational seminar to explain the effect of this waiver. III. Sign a “Waiver of Insurance” for discontinuing health insurance coverage with the Town.
