Reporting Sick Leave. It is the responsibility of the faculty member to ensure proper reporting of the use of sick leave for record keeping purposes. Any occurrence that necessitates a leave of absence as specified in Section 11.01 shall be reported by the faculty member to the Human Resource Office each month.
A. Abuse of sick leave occurs when a faculty member misrepresents the actual reason for charging an absence to sick leave, when a faculty member uses sick leave for unauthorized purposes, or when a faculty member neglects to report sick leave.
Reporting Sick Leave. 1. Except for emergencies, all requests for sick leave which can be anticipated (hospitalizations, surgical procedures, etc.) shall be submitted in advance. The request shall be made through the Employee Self Service system. The request shall be submitted to the appropriate Xxxx, or other appropriate management personnel, who shall indicate his/her approval or disapproval in a timely fashion.
2. When a FACULTY MEMBER experiences an emergency illness, it is incumbent on the FACULTY MEMBER to notify the appropriate Xxxx or other appropriate management personnel in a timely fashion.
3. The University personnel office shall maintain the official leave record of FACULTY MEMBERS in accordance with the provisions of Article 13, PERSONNEL FILES, and shall make periodic adjustments as appropriate to insure credit/usage records.
Reporting Sick Leave. (a) The employee shall notify the BCGEU as soon as possible of their inability to report to work because of illness or injury and shall make every reasonable effort to advise the BCGEU of the date of return to duty.
(b) An employee absent from work through illness or injury shall, within seven days from the initial day of absence, submit a fully completed sick leave application form. The BCGEU may request that a report from a qualified medical practitioner accompany the application for sick leave if the absence is over three working days. The BCGEU may also request a report from a qualified medical practitioner if it appears that a pattern of absence is developing. Any doctor's report or certificates requested by the BCGEU shall be paid by the BCGEU, upon the production of a receipt.
(c) An employee is not eligible for sick leave with pay for any period during which they are on leave of absence without pay, under suspension, on strike, on layoff or locked out.
Reporting Sick Leave. An employee in need of sick leave shall, in a timely manner, report such to his/her immediate supervisor.
Reporting Sick Leave. (1) To be paid sick leave, the employee has the responsibility to report their inability to be on the job. Failure to notify his office or Department Head prior to the established work time may result in loss of pay for that day and may result in disciplinary action against the employee. If notification is not possible prior to the start of the established work time, the employee shall notify the Department Head as soon as is possible.
(2) Sick leave time used will be subtracted on a hour‑for‑hour basis.
(3) Upon return to work the employee shall file a Notice of Leave form stating the reasons for the absence, unless the employee filed the form prior to taking Sick leave. The form must be approved by the Department Head before being charged to sick leave. Individual records of all sick leave credit shall be maintained by the County.
(4) A Department Head may require the employee to submit a medical statement, period of treatment, and date that the employee may return to work from sick leave when it occurs before or after a holiday or other scheduled day off or when an employee has a record of repetitious usage of short amounts of sick leave over an extended time period. The employee may be required to take a medical examination on returning from sick leave or on such occasions that it is in the best interest of the County. The medical examination shall be given by a physician designated by the County.
(5) The County Judge, an Elected Official or a designee may investigate the alleged illness of an employee under the Elected Official’s supervision absent from work on sick leave. False or fraudulent use of sick leave shall be cause for disciplinary action, up to, and including, dismissal.
Reporting Sick Leave. The Assistant Superintendent shall follow District procedures and use District forms for reporting sick leave use.
Reporting Sick Leave. (a) Prior to the employee's scheduled time to report for duty, the employee must notify the employee's office or immediate superior by telephone or messenger that the employee will not work that day.
(b) Upon return to work, the employee must file a Sick and Off-Duty Report. The Sick Report Form must be approved by the department head or designated representative before being charged to sick leave.
(c) The Physician's Report Section of the Sick and Off-Duty Injury Report must be submitted when the leave extends beyond five (5) consecutive calendar days (40 hours) or can be required if an employee has 48 or more undocumented sick leave hours in a calendar year. Employees are required to keep their supervisor appraised of their progress and anticipated return to work.

Reporting Sick Leave. In order for unit members to receive sick leave compensation, the procedures below will be followed: The unit member must notify the immediate supervisor or designee at the work location of his/her absence within the first hour of work of the first day of absence unless conditions make notification impossible. The unit member who has been absent five days or more and who has been notified that a substitute has been hired shall, at least one day prior to his/her expected return to work, notify his/her supervisor so that the employment of any substitute unit member may be terminated. If the unit member fails to notify his/her supervisor and both the unit member and the substitute report, the substitute is entitled to the assignment, and the unit member shall not receive pay for that day. An unit member absent for three (3) working days or more may be required to present a physician's statement stating the nature of the illness or injury and the date the unit member is able to return to work. The District will require verification of all absences if there is reason to know or suspect abuse of the personal illness and injury leave by a unit member. Any such request for verification shall be preceded by a written explanation and warning from the District that the unit member will be required to provide such certificate prior to any further sick leave usage. Any such written warning shall remain in effect for twelve
Reporting Sick Leave. Employees who will be on sick leave are responsible for telephoning not later than fifteen (15) minutes following their start time to report off to the immediate supervisor. Employees with assignment responsibilities commencing prior to normal agency starting times must report off early enough to allow for timely coverage. Subsequent reports should be made each day unless the nature of the illness/injury is such that an estimated time of absence can be projected. Individual supervisors may develop more specific guidelines for reporting off for their work units. Upon returning to work after use of sick leave, an employee must complete and sign and “Application for Leave” form to justify use of sick time. The completed form is to be forwarded to the Personnel Office. In cases of extended absence, the form may need to be completed prior to the return to work. The Agency reserves the right to request a doctor’s statement when:
A. An employee has a sick leave bank of one hundred twenty (120) hours or more and the employee’s sick leave extends beyond three (3) days. Such sick leave bank requirement will be prorated for part-time employees.
B. When Family and Medical Leave (FML) is being requested/charged.
C. An employee has a sick leave bank of less than one hundred twenty (120) hours and the employee’s sick leave extends beyond two (2) days. Such sick leave bank requirement will be prorated for part-time employees.
D. A doctor’s statement may also be requested to release the employee to return to the job after a serious illness or injury.
E. Additionally, whenever the Employer has provided an employee with written notice of excessive and/or patterned absence, inclusive of expected improvement, the Employer may require that any future requests for sick leave by said employee be accompanied by a signed statement from a licensed physician in order to justify the use of said leave. Excessive/patterned absence may also be cause for discipline. The Employer is committed to consistency under similar circumstances. Sick leave is substituted for unpaid Family and Medical Leave whenever an employee seeks the benefits/protections of the Family and Medical Leave Act and for absences which qualify under FML. Family and Medical Leave is available to eligible employees generally for absences of three (3) consecutive work days or more, or for intermittent leave as certified as medically necessary.
