Responsibilities of the Resident. 6.1. The Resident will:
a) keep the Unit and Room in a clean and sanitary condition;
b) be responsible for reading, acknowledging, and strictly abiding by the Residence Conduct Guide posted on the Residence Services website, as modified from time to time;
c) be responsible for their own personal property;
d) be responsible for reading and abiding by the ResNet Acceptable Use Policy which can be found on the Residence Services website at xxx.xxxxxxx.xx;
e) permit MRU or its agent entry for the purpose of inspecting the condition of the Unit and Room and its contents and for reasons described in the Resident Conduct Guide;
f) be solely liable to MRU for any loss or damage to the Room and the furniture and effects of MRU within;
g) be liable jointly and severally with the other Residents of the Unit for any loss or damage to the Common Areas of the Unit and any furniture and effects of MRU within;
h) be liable jointly and severally with any other occupants of the University Residence for any loss or damage in the Common Area of the University Residence and any furniture and equipment of MRU;
i) be liable for the conduct and damages, if any, of its guests to the Unit, the Room or the University Residence;
j) report any damage or vandalism or report persons known to be responsible for damage or vandalism as soon as practicable to Residence Services; and
k) download the MRU Now application and enable push notifications to keep aware of serious issues requiring immediate attention
6.2. The Resident will not:
a) do or permit or allow anything to be done in the Unit or the Room which is illegal or which is a nuisance or an annoyance to any occupant of the Unit or of the University Residence or any member of Residence Services;
b) damage the Unit or the Room or any furnishing or fixture in the Unit or Room, or abuse any service in the Unit or Room;
c) affix anything on the walls, floor, ceiling, doors or windows of the Unit or Room which may disfigure or xxxx the surfaces when they are removed;
d) apply any materials or utilize markers on any surface, including windows, make any alteration or renovation to the Unit or Room;
e) fail to close windows when the exterior temperature is below zero degree Celsius;
f) alter or overload an electrical component, including but not limited to outlets, panels or wiring
g) Do or permit any form of soliciting, surveying polling or commercial activity, whether selling, advertising or otherwise, throughout the complex; or

Responsibilities of the Resident a. Lessee and Lessee’s household members and guests shall not conduct themselves in a way that unreasonably disturbs other residents, and shall refrain from illegal activities. Lessee shall be responsible for the activities and behavior of persons residing with Lessee and guests of Lessee.
b. Lessee shall not use or occupy his/her mobile home in such a way as to cause any lot to fail to comply with the terms of this Lease Agreement or State or local laws and ordinances. The resident, members of the household, guests and invitees shall not deliberately or negligently destroy, deface, damage, alter or remove any fixture, mechanical or utility system, or furnishing of the Park or any resident.
c. The Lessee shall be responsible for the expense of maintaining the plumbing, electrical, and other utility service within the home, and from the point at which the service surfaces under the mobile home or connects to the mobile home from a service point.
d. Lessee shall maintain his/her unit free from rodents and reasonably free from insects, vermin and other pests.
e. The Lessee is responsible for the maintenance of the lot, including but not limited to the maintenance of the lawn and driveway, including snow removal from driveway and parking areas.
f. The lessee is responsible for any damage caused by failing to control water leaks within the mobile home or disposing of anything other than normal domestic water into the sewage system that may cause blockages, surfacing or backup. In the event of water or sewage leaks, contact the Park Manager immediately. The Lessee shall not interfere with the main water control valve to the park. Any fine incurred by the Lessor due to a Lessee interfering with water utilities shall be billed to the Lessee.
h. Upon the termination of the lease, the Lessee will leave said premises in as good a state of condition as they were at the beginning of the lease.
i. Lessee is responsible for the payment of property taxes resulting from the installation of Lessee’s mobile home on the Lot. Lessee is responsible for paying property taxes directly to the Tax Collector.
Responsibilities of the Resident. The resident will read, understand and intend to comply with the Regulations Governing the Practice of Licensed Substance Abuse Treatment Practitioners in Virginia. The resident will verify that the supervisor has been approved as a supervisor by the Virginia Board of Counseling. The resident will establish and execute a supervisory contract for supervision before providing clinical substance abuse treatment services as a Licensed Resident in Substance Abuse Treatment and before counting hours toward LSATP licensure. The resident will participate in supervision with a goal of increasing competency in clinical practice and attend supervision on the agreed upon basis. The resident will be prepared to discuss with the supervisor the diagnosis/case conceptualization and treatment of each client as well as problems in the resident’s clinical milieu. The resident will provide appropriate and current raw data and clinical materials for supervision which are representative of the resident’s practice or of the specialty where more guidance and direction is needed. The resident will avoid engaging in activity for which the resident lacks competency, training, education, supervision and which may compromise client safety and wellbeing. The resident will refrain from calling themselves a substance abuse treatment practitioner or in any way representing themselves as an independent, autonomous practitioner or Licensed Substance Abuse Treatment Practitioner. The resident will refrain from directly billing for services rendered. The resident will, during the residency, include the resident’s name and the initials of resident’s degree, resident license number, and the title "Resident in Substance Abuse Treatment" in all written communications. The resident will inform clients in writing that the resident does not have authority for independent practice and is under supervision. The resident will provide the supervisor's name, professional address, and phone number. The resident must renew their temporary Resident in Substance Abuse Treatment license each year and will be required to complete three hours of continuing education courses that emphasize the ethics, standards of practice, or laws governing behavioral science professions in Virginia. Responsibilities of the Resident. The Resident will keep the Pet in the Suite, except when walking the Pet, if applicable, or transporting it to and from the Suite. The Resident will not allow the Pet in building lobbies or in common residential area, other than to transport the Pet to and from the Suite. The Resident will walk and curb the Pet only in areas designated by the Community and will be responsible for cleaning up after the Pet. When the Pet is not in the Suite, the Resident will keep it on a leash no longer than five feet or in a cage or other appropriate closed and ventilated container, and in the control of the Resident. If the Pet is a bird, the Resident will keep it caged both in and out of the Suite. If the Pet is a dog or cat, the Resident will ensure that it wears a collar with appropriate identification (including the Resident’s telephone number) at all times that it is out of the Suite. The Resident will comply with all vaccination and licensing requirements applicable to the Pet, showing proof of this upon request, and will comply with appropriate standards of care, treatment, and grooming. The Resident will be responsible for the health, welfare, and proper care of the Pet. The Resident will ensure that the Pet does not disturb the right of other residents to the peaceful enjoyment of their Suites and of the common areas. The Resident will not leave the Pet unattended when the Pet is not in the Suite. The Resident will be liable for any personal injury or property damage caused by the Pet that is suffered by the Community, its employees or agents, other residents, guests, or invitees. The Resident will pay all costs and expenses, including reasonable attorneys’ fees and court costs incurred by the Community in enforcing any liability of the Resident under this Addendum.

3.2 The resident agrees to participate fully in the educational and scholarly activities of the residency program and, as required, to assume responsibility for teaching and supervising other residents and students of the Academic Health Center.
3.3 The resident agrees to provide safe, effective, and compassionate care of patients under faculty supervision, commensurate with the resident’s level of education and experience.
3.4 The resident agrees to abide by the bylaws, policies, rules, and regulations of the University of Minnesota School of Dentistry (the “School of Dentistry”), the University and the hospital and clinics to which assigned.
3.5 The resident agrees to meet state, federal, and University requirements for participating in a residency program prior to the start of and throughout the training program. These requirements include: a) credentials - submit proof of earning a D.D.S. or D.M.D or equivalent degree, comply with state licensure requirements by obtaining a resident dentist license from the Minnesota Board of Dentistry if not already fully licensed to practice dentistry in the State of Minnesota as required by the residency program; b) health professional requirements - submit proof of immunization; complete a background study request under Minnesota law; complete HIPAA training; c) work authorization - obtain an appropriate visa, as agreed to by the program, if the resident is not a U.S. citizen or permanent resident; and d) other - complete a Human Resources Information Form (HRIF) upon appointment; and comply with any other requirements established by the residency program. The resident agrees that their immunization compliance status and background study results may be shared with clinical sites where the resident is assigned.

Responsibilities of the Resident. As of the Commencement Date, the Resident must satisfy all credentialing and pre-employment obligations prior to entry into the Program and having done so is a full time Resident of THMEP, and agrees to devote his/her full professional time and attention to the performance of his/her duties hereunder for and on behalf of THMEP. The Resident will be scheduled to work according to a schedule made by THMEP and published in advance and shall satisfy the goals and objectives of the Program as set out in any Addendum hereto. The Resident may be asked to work additional hours in order to ensure patient needs are met and full coverage is provided in the department. The Resident agrees as follows:
1. To perform satisfactorily and to the best of his/her ability, the customary responsibilities of a Resident.
2. To participate in safe, effective and compassionate patient care under supervision from the program.
3. To participate fully in the educational activities of the program and, as required, assume responsibility for teaching other residents and/or students.
4. To develop a personal program of self-study and professional growth with guidance from the preceptors.
5. To adhere to established practices, procedures and policies of the participating institutions.
6. To participate in program committees and activities, especially those that relate to patient care review and continuous quality improvement.
7. To apply cost containment measures in the provision of patient care.
8. To complete the rotation evaluation forms and other documents requested at the conclusion of each rotation.
9. To protect oneself and one's patients by consistently and conscientiously observing universal precautions and other infection control measures, including self-immunization against hepatitis B.

No Resident, any member of Resident's household, a guest, a visitor, any individual on the premises through any association with the Resident or any person under the Resident's control shall use, possess, manufacture, distribute, or sell any illegal drug or controlled substance (as defined in section 102 of the Controlled Substance Act, (21 U.S.C. 802)) of any kind or nature. Furthermore, no Resident, any member of Resident's household, a guest, a visitor, any individual on the premises through any association with the Resident or any person under the Resident's control, shall engage in any activity whatsoever connected and/or associated with the use, possession, manufacture, distribution or sale of any illegal drug or controlled substance (as defined in section 102 of the Controlled Substance Act (21 U.S.C. 802)) of any kind or nature. Any single incident of such aforementioned conduct by Resident, or any member of Resident's household either within Resident's premises as well as on or off the premises, not just on or near the premises, shall be adequate grounds and good cause for termination of this dwelling lease (without further opportunity to cure or remedy such activity) and removal of Resident and all members of Resident's household from Management properties by proceedings in a court of proper jurisdiction. In appropriate cases, the Management may permit continued occupancy by remaining family members and may impose a condition that family members who engaged in criminal-related drug activity as described above will not reside in the unit.
2. Resident shall have the right to exclusive use and occupancy of the dwelling unit for Resident and other household members listed on the lease. With prior written consent from Management, members of the household may engage in legal profit-making activities in their dwelling unit. Resident shall conduct such profit-making activities in accordance with the Louisville Metro Housing Authority Admission and Continued Occupancy Policy.
3. Visitors and guests of the Resident must limit their stays to fourteen (14) calendar days per year whether or not such days are consecutive. Visits longer than fourteen (14) days are permitted only with the prior written approval of Management. Management has the right at any time to require Resident to verify in writing any guest’s permanent residence.
