RESPONSIBILITIES OF THE UNIVERSITY. The UNIVERSITY shall designate in writing a faculty member to coordinate with a designee of the FIELDWORK SITE.
RESPONSIBILITIES OF THE UNIVERSITY. The University shall:
(1) outline the course of study for the Students, with a view to developing in each Student the skills and knowledge as outlined by the University’s curriculum;
(2) require Students assigned to the Placement meet academic pre-Placement requirements established by the University prior to the Placement;
(3) appoint an Advisor whose responsibilities are:
(a) to evaluate the Students' performance in accordance with the academic objectives/learning outcomes established by the University and in consultation with the staff of the Practicum Centre; and
(b) in cooperation and with the assistance of the Practicum Centre, to inform students that they must be oriented to and be familiar with the Practicum Centre’s mission, rules, regulations, and policies of the Practicum Centre; provided that in the exercise of these responsibilities the Advisor is subject to the operational requirements of the Practicum Centre;
(4) inform Students they must comply with and abide by the Practicum Centre’s policies and procedures, and the instructions of the supervisor and her/his designate as supervisor of the Students, during the term of the Placement, and inform the Students that during the term of their Placement and thereafter they are required to abide by the Practicum Centre’s Conflict of Interest policy and confidentiality requirements, and are subject to the provisions of legislation governing the Practicum Centre; and
(5) assign the overall grade for each Student’s Placement and assume overall responsibility for the Students’ academic program.
RESPONSIBILITIES OF THE UNIVERSITY a. The University agrees to exercise reasonable care to safeguard the health, safety, and property of the Resident and will make a good faith effort to repair properly reported defects in residential facilities. The University shall not be liable for failure of water supply, electrical current, or heating/cooling systems; presence of insects or vermin; or the loss, damage, or injury to a resident or his or her guest or the property of the Resident or guest. In the event of utility or facility disruptions, housing fees will not be reimbursed.
b. The University only provides liability for damages or injuries caused by negligence on the part of the University or its employees while working within the scope of their employment. THE UNIVERSITY IS NOT LIABLE FOR DAMAGE OR LOSS OF PERSONAL PROPERTY. STUDENTS ARE STRONGLY ENCOURAGED TO SECURE THEIR OWN PERSONAL PROPERTY LOSS INSURANCE.
RESPONSIBILITIES OF THE UNIVERSITY. The Medical School shall be responsible for providing a graduate medical educational experience and training program through faculty planning, teaching, supervision, and evaluation of residents/fellows. The University agrees to perform administrative functions for the benefit of the residents/fellows. These include arranging for the payment of stipends; maintaining certain resident/fellow records; administering the procedure related to the discipline of residents/fellows; and providing mechanisms for the coordination of programs among the affiliated hospitals and clinics, the Medical School, and the various clinical services. The University agrees to provide the following benefits to residents/fellows effective the date set forth in Section 1.1: Benefits as set forth in the GME Institution Policy Manual. All residents/fellows receive the following benefits regardless of appointment classification: professional liability insurance through the University covering claims related to duties performed as part of the residency/fellowship, whether such claims arise during or after the resident’s/fellow’s completion of the program; other insurance through the Office of Student Health Benefits specifically for residents/fellows that provides medical insurance and optional dependent coverage, long-term and short-term disability insurance, life insurance, and optional dental insurance at the resident’s/fellow’s cost; reasonable accommodations for residents with a documented disability as outlined in the Institution Policy Manual; and counseling and psychological support services through a residency/fellowship assistance program, including monitoring and assistance for impaired physicians consistent with professional and legal obligations. Residents/fellows who are not appointed in a training grant position are eligible to participate in the University’s dependent and health care flexible spending accounts and the Optional Retirement Plan (ORP) or the University of Minnesota Section 457 Deferred Compensation Program (457 Plan). Leave of absence benefits, which include parental/family medical, professional/academic, personal, vacation, holiday, sick, bereavement, military and jury duty/witness leave. RESPONSIBILITIES OF THE UNIVERSITY. 4.3.1 The University shall carry out Quality Monitoring to ensure that the Services are being delivered at the performance and quality levels set out in the Schedules herein. The Practice shall co-operate with the University and provide the University with such assistance as is requested by the University in order for the University to undertake the Quality Monitoring. Quality Monitoring shall not relieve the Practice of any of its obligations under this Agreement.
4.3.2 In consideration of the Practice providing the Services, the University shall make payments to the Practice in accordance with Schedule B.
RESPONSIBILITIES OF THE UNIVERSITY. The University shall take reasonable steps to ensure that the student is made aware of the policies, rules and regulations of the Organization as made known to the University. The University shall provide the Organization with the University’s policies, rules and regulations that are necessary for the Organization to effectively participate in the Practicum. Further, as reasonably possible, the appointed representative of the University will meet with the Organization to review the University’s and the Organization’s applicable policies, rules and regulations and to clarify any questions either party may have regarding such policies, rules or regulations. The University shall pay the Organization or a representative of the Organization an honorarium as set out in Schedule “A”, provided the Organization has complied with the terms of this Agreement including the requirement to provide the student with adequate and consistent supervision as set out in section 5.4 herein.
RESPONSIBILITIES OF THE UNIVERSITY. 1. The University shall provide the basic academic preparation of the student interns through classroom instruction and laboratory practice and will assign to the Facility only those student interns who possess a satisfactory record of completing prerequisite portion of the curriculum and who have met the minimum requirements established by Facility for the Program.
2. The University will maintain general responsibility for didactic instruction, academic evaluation and related academic matters concerning student participation in the educational internship program at the Facility, including evaluation and grading of student interns.
3. The University will provide to appropriate personnel at the Facility a list of the student interns to receive training at the Facility and will update such list as necessary.
4. The University shall advise student interns that their participation in the training experience does not entitle the student interns to employment with the Facility that extends beyond their participation in the internship at the Facility.
5. The University will promptly consider any reasonable request by the Facility for the withdrawal of student interns for sufficient and good cause if they are participating in an unpaid internship. The University acknowledges that student interns participating in paid internships are at will employees of the Facility, and that the Facility has the right, in compliance with applicable laws, to terminate the student interns at any time, with or without cause.
6. The University shall provide the Facility’s staff with opportunities to participate in the development of specific educational objectives for each student intern as well as in the joint planning and evaluation of the student intern’s educational experience.
7. The University is an agency of the State of New Jersey. Any agreement signed on behalf of the State of New Jersey by a State official shall be subject to all of the provisions of the New Jersey Tort Claims Act (N.J.
RESPONSIBILITIES OF THE UNIVERSITY. A. Ensure information pertaining to Career and Workforce Development Dual Enrollment is made available to students on the FGC’s website.
B. Will design and develop content for Career and Workforce Development Dual Enrollment offerings.
C. Responsible for review, selection, appointment, administration, evaluation, and coordination of offering directors, faculty, and staff.
D. Responsible for the following student matters: registration, collection of fees, distribution of books, and reporting.
E. Shall provide and maintain the Learning Management System (LMS), as applicable, for presentation of Career and Workforce Development Dual Enrollment offerings to students.
F. Publish deadlines and procedures on the Career and Workforce Development Dual Enrollment website.
G. Notify the student of their grades. In addition, a document will be sent electronically to the High School indicating work completed.
H. Have a process in place for virtual instructors to comply with student IEPs and 504s as well as serve the needs of English Language Learners.
I. Assign a letter grade to each student enrolled in a Career and Workforce Development Dual Enrollment course. The letter grade assigned by the postsecondary institution shall then be posted to the high school transcript by School Board pursuant to s. 1007.271(20), F.S.
J. Be responsible for making an annual report to the Commissioner of Education on the operation of the Career and Workforce Development Dual Enrollment program. County Public Schools will provide FGC any information requested to complete such reports.
K. Be responsible for monitoring the quality of curriculum to ensure that instruction is consistent with the Florida Gateway College’s policies and procedures.
RESPONSIBILITIES OF THE UNIVERSITY. A. The UNIVERSITY will assure that the student shall have completed the necessary educational prerequisites, to be eligible for supervised fieldwork including proof of negative TB test current within one year of supervised fieldwork and issuance of finger print clearance.
B. The UNIVERSITY shall designate a faculty or staff member to coordinate, consult, and collaborate with the classroom teacher or district designee of the FIELDWORK SITE, the activities of each student assigned to FIELDWORK SITE and student fieldwork experience.
C. The UNIVERSITY shall complete periodic observations and/or evaluations of the student regarding his/her performance at the FIELDWORK SITE as per arrangement between the UNIVERSITY faculty or staff member and the FIELDWORK SITE supervisor.
D. The UNIVERSITY may provide monetary compensation for services rendered by the FIELDWORK SITE in an amount not to exceed the actual cost of the services rendered by the FIELDWORK SITE per Appendix A.
RESPONSIBILITIES OF THE UNIVERSITY. A. The University will assume responsibility for developing and implementing the educational program in physical therapy.
B. The University will refer to The Facility only those students who are enrolled in the University’s physical therapy curriculum and for the full-time clinical internships, those who have satisfactorily completed the academic prerequisites for clinical education experience per program matriculation requirements. (See Physical Therapy Clinical Educator’s Handbook.)
C. The University will designate a person or persons to direct the clinical education programs at the University and to act as liaison for the University, The Facility and the student(s). D. The University will be responsible for the determination of a student’s final grade for clinical education experiences, feedback from The Facility evaluation forms will be used in making this determination.
E. The University will notify The Facility of its planned schedule of student assignments, including the dates of full-time clinical experiences, the name(s) of the student(s), and the level of academic and preclinical preparation of each student.
F. Upon execution of this contract, the University will provide The Facility with educational objectives and evaluation forms for each clinical education assignment.
G. The University will maintain communication with The Facility on matters pertinent to clinical education. Such communication may include, but not be limited to, on-site visits to The Facility, workshops, meetings, and the provision of educational materials relevant to the clinical education program.
H. The University will advise students assigned to The Facility of their responsibility for complying with not only the existing rules and regulations of The Facility, but also policies and procedures of The Facility as well, including but not limited to, complying with any physical examination/immunization requirements of The Facility.
I. The University will maintain professional liability insurance for each student assigned to The Facility and will provide The Facility with information regarding such liability insurance. The specific insurance limited will be as outlined in the provided student liability insurance document.
