Hospitalization/Major Medical. 7.01.1 For all employees hired prior to August 1, 2007, the Board shall provide single and family coverage for all certificated/licensed employees of hospitalization insurance and major medical that provides no less benefits than the 1/1/2019 POS II Plan (see Exhibit “B”). Employees hired prior to August 1, 2007, shall have the option to elect coverage under the POS II Plan or to participate in the High Deductible Plan/Health Savings Account option, outlined in 7.02 below.
7.01.2 All part-time employees (working under 30 hours per week) will pay a pro- rated share of the premiums for hospitalization, major medical (whether POSII or HD/HSA) and dental insurance which they opt to accept as part of the fringe benefit package offered by the Board. Current part-time employees receiving benefits will be grandfathered from this provision until the beginning of the 2016-17 contract year. The Board will not split full-time positions in order to reduce a benefit eligibility by instead hiring part-time employees. Teachers may still opt to job-share pursuant to Section 5.11.
7.01.3 For the duration of this agreement, the Association and the Board shall have an identified insurance committee whose members shall be appointed by the President of the Association and the Superintendent of the Chardon Local School District. The District Treasurer shall serve as chair. Four (4) teacher members shall be paid at a rate of thirty dollars ($30) per hour. Primary duties of the insurance committee shall include but not be limited to topics concerning major medical expenses, premiums, insurance carriers, Section 125 plans, wellness programs, legalities, etc. On or before May 1st of each year of this agreement, the committee will examine the current health plans, including the High Deductible Plan/Health Savings Account option, and cost to determine if a change is needed. Upon the recommendation of the Insurance Committee, the Board may change carriers provided all benefits remain equivalent to the 1/1/2019 POSII/HSA Plan. When insurance is projected to increase by at least ten percent (10%) per year, the insurance committee shall convene to discuss and recommend how to bring the projected increase under ten percent (10%).
7.01.4 The Board and the Association agree that each employee in the POSII Plan shall contribute twenty percent (20%) of the insurance premium.

Hospitalization/Major Medical. The Board shall provide single or family insurance coverage for all bargaining unit employees as provided below.
1. The Board shall pay eighty percent (80%) of the full premium amount for eligible full-time employees (30 hours or more per week) hired prior to July 1, 2007, and said employees will pay twenty percent (20%) of health insurance premiums. Employees in the POSII Plan who complete basic annual biometric screening will pay $200 less for family coverage or $100 less for single coverage per year for employee healthcare contributions. If enrolled in the family plan, both employee and spouse must complete biometric screening.
a. The co-pay for office visits shall be fifteen dollars ($15.00).
b. The deductible costs shall be $400.00 for family coverage and $200.00 for single coverage.
2. Full-time (i.e., 30 or more hours of work per week) employees hired prior to July 1, 2007, shall have the option to elect coverage under the POS II Plan or to participate in the High Deductible Plan/Health Savings Account (HD/HSA) option, outlined in Subsection 3., below without the obligation to remain under said option subject to open enrollment limitations. Employees covered under the POS II Plan shall be entitled to all of the benefits outlined in Section F., below, titled “Section 125 Plan.”
3. For all full-time employees (defined as thirty [30] hours or more per week) hired on or after July 1, 2007, the Board shall offer single and family coverage for all employees for hospitalization insurance and major medical pursuant to the District’s High Deductible/Health Savings Account (HD/HSA) option. Any employee covered under the HD/HSA option shall pay five percent (5%) as an employee contribution.
a. The HD/HSA shall be a $2,000.00 aggregate deductible for single coverage and a $4,000.00 aggregate deductible for family coverage.
b. Effective January 1, 2023, for each employee who elects single coverage in the HD/HSA, the Board shall contribute $800.00 annually toward a Health Savings Account (HSA) for a full-time employee providing that the employee completes basic annual biometric screening. Employees electing to forego the basic annual biometric screening will receive $700 annually toward an HSA.
c. Effective January 1, 2023, for each employee who elects family coverage in the HD/HSA, the Board shall contribute $1600.00 annually toward a Health Savings Account (HSA) for a full-time employee providing that the employee and spouse complete basic annual biometric scre...
Hospitalization/Major Medical. The School Board will participate in the hospitalization/major medical insurance program by paying for full-time teachers the following amounts toward the monthly premium costs of any plan approved by the School Board. Any additional cost of premium shall be borne by the employee and paid by payroll deduction. For the purpose of this section, full-time teacher shall be a teacher whose average work-week equals or exceeds 37.5 hours. Insurance contributions/cafeteria benefits from the district will continue for a period up to one (1) year while on workers compensation leave.
Hospitalization/Major Medical. 25.01 The Employer shall select the carrier for the insurance programs herein. However, any change in carriers or programs will not reduce any of the current levels of benefits in force and effect as of, the date of ratification, as described in Appendix E unless otherwise mutually agreed between the parties as refer- enced below.
25.02 Employees of the Bargaining unit in full-time active pay status, or approved Family and Medical Leave, are eligible to participate in the health insurance plan provided to the Employer’s non-bargaining employees for the duration of this Agreement, as provided below:
A. Any change in carriers or programs, as recommended by the Cost Containment Commit- tee and approved by the Board of Erie County Commissioners will amend this agreement to reflect said change;
B. Upon execution of this agreement, employees will be required to contribute at the same rate as non-bargaining unit employees provided that the premium contributions do not ex- ceed 13% in 2018; 13.5% in 2019; and 14% in 2020 of the total cost of the premium as recommended by the cost containment committee and approved by the Board of Commis- sioners, and/or required by law. Employees will be eligible to participate in the contribu- tion credit program;
C. The mandatory spousal enrollment provision of the Health Insurance Plan will not be im- plemented during the life of this contract; and,
D. The Union’s designee will continue to be a member of the County’s Cost Containment and Wellness Committee and may attend these meetings on County time if during the em- ployee’s regularly scheduled shift; Attendance shall not be unreasonably denied nor shall it generate overtime.
25.03 Insurance Booklets shall be provided to all eligible employees. Eligibility for medical, vision, den- tal and life insurance begins after ninety (90) days of employment. The current Master Plan Document will be available on-line at xxx.xxxxxxxxxx.xx.xxx. Eligibility for insurance benefits begin the 1st of the month following 30 days of employment, e.g., an employee hired June 5 will be eligible for insurance August 1.
Hospitalization/Major Medical. The Employer shall pay the premiums for hospitalization coverage for the member of E.
Hospitalization/Major Medical. The Board shall provide single person and family hospitalization, medical and major medical coverage for all full- time unit members as outlined below. The Board will pay a percentage (%) of premium for coverage for a bargaining unit member contracted for at least 30 hours per week equivalent to their FTE less the employee contribution amount with the unit member picking up the balance. Employees working less than 30 hours per week are not eligible for the District’s health insurance coverage. The Board will pay its proportionate cost of the premium for one (1) family health care plan or two (2) single health care plans for any married couple when both are employees of this Board. The health care plan shall include coverage for hospitalization/major medical, prescription, vision, and dental care.
Hospitalization/Major Medical. The Board shall purchase through a carrier licensed by the State of Ohio, a Plan for hospital/major medical insurance coverage. Cost of such insurance shall be as follows for the duration of this contract. The Board’s contribution toward the monthly premium will be 87.5% for full-time employees; 82.5% for bus drivers; and 77.5% for 25-34 hour employees. OAPSE’s contribution toward the monthly premium will be 12.5% for full-time employees; 17.5% for bus drivers; and 22.5% for 25-34 hour employees. The Board will offer any/all insurance plans to the Bargaining Unit that are offered to other District Bargaining Units. The OAPSE Local #775 may have up to 4 representatives on the Health Insurance Committee. The Health Insurance Committee will continue to investigate and evaluate insurance options on a yearly basis and report to the Board. The Board and/or the Association can make recommendations to the Health Insurance Committee. Any recommendations from the Health Insurance Committee will be presented to the association and the Board. The Board has final approval of the recommendations made by the Health Insurance Committee or may require the committee to seek out other options. Current employees eligible for benefits under a (20-34 hour classification) of the 2006-2009 contract will be grandfathered in at their current rates of (77.5% Board paid 22.5% Employee paid) for the duration of this contract.
Hospitalization/Major Medical. 1. The Board shall purchase and make available to members of the bargaining unit a PPO health care plan that includes prescription drug coverage. Insurance coverage and benefit levels will be maintained during the term of this Agreement (A) except as otherwise may be mutually agreed by the Board and the Association, and (B) except as required to maintain compliance with federal and Ohio law. The Board shall implement a Section 125 tax shelter plan covering each teacher’s contribution toward the cost of monthly health care benefits. The Board shall comply with IRS requirement.
2. The Board will pay 90% of the premium cost for individual hospitalization/major medical coverage. The Board will pay 80% of the premium cost for family coverage.
3. Effective with January 2020 coverage, the Board will offer two (2) alternative medical insurance plans in addition to the current plan referred to in Paragraph 1 above. An employee who chooses to participate in one of these alternative plans will receive (A) either the same dollar contribution by the Board for the monthly cost of the chosen plan as the Board would have paid if the employee had elected to participate in the current plan, or (B) 100% of the monthly cost of the chosen plan if such cost is less than the previously stated dollar contribution.
Hospitalization/Major Medical a. The Board shall pay eighty percent (80%) of the insurance premium and the member shall pay twenty percent (20%) of the insurance premium for hospitalization/medical insurance. In the event the premium amount increases by more than fifteen percent (15%) during the term of the negotiated labor agreement, the Board and the member shall split the amount of increase greater than fifteen percent (15%) equally. The Board shall implement the Anthem Plan change effective October 1, 2011.
