School District Responsibilities. The Shaler Area School District agrees to comply with all related federal, state, and local laws pertaining to the delivery of mental health and drug and alcohol rehabilitation services within school districts, including but not limited to the Family Education Rights and Privacy Act (FERPA) and the Protection of Pupil Rights. The school district also agrees to provide a SAP team that complies with the BEC 24 P.S. 15-1547 for membership, training, common planning times, and ongoing maintenance. Additional responsibilities of the school district include: 1. The school district will designate a contact person between the team and the provider to ensure effective communication. The school district’s contact will Xx. Xxxxxxxx Xxxxxxx and can be reached at (412) 492-1200 ext. 2815. 2. The school district will appropriate a safe and private space in the school where the SAP liaison can provide services and adhere to SAP confidentiality provisions. 3. The school district will provide copies of the district’s alcohol, tobacco, and other drug policy, suicide/mental health crisis policy, school calendar, a schedule of special activities, and any other school policies, which may affect Student Assistance Program services. 4. The school district will provide family and community education on the Student Assistance Program. 5. The school district will provide faculty, pupil personnel and student orientation to the Student Assistance Program that includes staff, services, and referral procedures. 6. The school district will provide release time as established by the core team for referred students. Release time shall coincide with the normal school day and will be designed so that instructional time is not abused. 7. The school district will contact parents or guardians of identified students in order to explain referral, gather information, and obtain permission to involve students in the Student Assistance Program. 8. The school will submit data (on-line reporting) regarding the Student Assistance Program as requested to the Departments of Health, Education, and Public Welfare.

Access to a properly lighted private office, which shall contain a telephone, a secure computer and printer, which may be used for general business purposes. 2. A location for files and records which can be properly locked and secured. 3. A desk with drawers, chair, work table, filing cabinet, and office supplies. 4. The opportunity for SROs to address teachers, school administrators and student families about the SRO program, goals, and objectives. 5. The opportunity to provide input regarding criminal justice problems relating to students. 6. The opportunity to address teachers and school administrators about criminal justice problems relating to students during in-service workdays. 7. The District Emergency Operations Manual, Crisis Plan, Student Handbook/Code of Conduct and other related materials as deemed appropriate. 8. School staff designee for referrals for counseling and other school-based and/or community based supportive services for students and families. 9. SROs shall respect the sensitive nature of student privacy and shall abide by all applicable confidentiality, privacy policies, and applicable laws. 10. Encourage attendance for secondary Assistant Principals at NASRO Basic SRO training. 11. Provide training to teachers, administrators, staff and SROs about when to directly involve SROs with student misconduct and about available alternatives to arrest.
School District Responsibilities a) The School District shall employ a maximum of two (2) Employment Specialists to provide services under this Arrangement. Each ES shall spend 100% of their time providing employment services described in this Arrangement to VR SE IPE students. The services provided by the ES shall not be those typical or customary services provided by the school district but must be new or expanded services made possible under this Arrangement. All services shall be based on the individual needs of the student and provided by the ES pursuant to the student’s needs. b) The School District shall designate a representative to act for the School District in all matters pertaining to this Arrangement. c) The School District shall request and obtain written approval from DVR before allowing the ES to provide CBWE services. d) The School District shall verify/provide documentation the ES is an employee of the School District. e) The School District shall submit the School District personnel action form (from the Human Resource Department), with the ES name, hire date, actual salary and terms of employment to the DVR Representative (Contract Manager). f) The School District shall provide a copy of the student’s current Individual Educational Plan (IEP) and each IEP thereafter while participating in a VR IPE to the VR Counselor. g) The School District shall provide non-federal match requirement to DVR in the amount of $8,520.00 for each ES that will be providing service under this Arrangement. h) The School District shall process and submit all reports, invoices and supporting documentation using the Rehabilitation Electronic Billing Application TPCA (XXXX TPCA), unless given written authorization by DVR to use an alternative method of approval for TPCA services. i) The School District shall make all requests for technical assistance in writing to the DVR Contract Manager listed in Section III. j) The School District shall maintain documents to support deliverables in a safe and secure location to provide for the integrity of the records, the student’s safety and confidentiality. These records and their location are subject to inspection and must be made available for review upon request.

Provide the services of teachers (as needed), and a Coordinator of Career & Technical Education or Local Vocational Education Coordinator, Curriculum Director, or Department Chair of Academic Department to serve as the program coordinator and liaison with MADISON COLLEGE administration. b. Arrange for MADISON COLLEGE course registration of all students involved in the program. Registration forms must be received by the MADISON COLLEGE Center for College & Career Transitions no later than thirty (30) days after the beginning of the contracted class. Failure to meet this deadline entitles MADISON COLLEGE to give a 7-day notice of cancellation of the Agreement as applied to any course for which registration forms have not been received. If the SCHOOL DISTRICT does not cure its failure to submit registration forms within the 7-day grace period following notice, this Agreement may be terminated as applied to the affected course(s) and students will not receive dual credit through MADISON COLLEGE. By mutual agreement between MADISON COLLEGE and the SCHOOL DISTRICT, registration forms for individual students may be accepted after the original deadline, provided that the Agreement covering the given course has not been cancelled in its entirety. c. Pay salary and fringe benefits of all SCHOOL DISTRICT employees working in the program according to the labor agreement between the SCHOOL DISTRICT and the Education Association. d. Provide adequate school classrooms and other facilities for the courses. e. Develop and implement procedures for screening and approving students for enrollment into courses. f. Provide school counseling (i.e., career guidance) services for students enrolled in or seeking enrollment in dual credit courses. g. Facilitate on-campus student experiences at MADISON COLLEGE. h. Provide MADISON COLLEGE with grade and other appropriate reports, and establish an agreed upon mechanism for collecting or sharing student demographic client reporting information that is consistent with pupil records laws. i. Establish high school credit value granted to students completing MADISON COLLEGE Dual credit courses. The SCHOOL DISTRICT reserves the right to apply SCHOOL DISTRICT grading criteria for purposes of determining high school credit and to give students the grade they earn in high school on the high school transcript (which may or may not be the same as the grade earned for the MADISON COLLEGE credits). j. School District Responsibilities. The laws of the State of Minnesota have vested in the School District the full authority and power to manage, control and direct the operation of the School District and to adopt, modify or repeal policies, rules and regulations for the school district. All such authority and power of the School District shall continue unimpaired, except as limited by the specific provisions of this Agreement and applicable State and Federal laws.
School District Responsibilities. School District will: A. Comply with the terms of this Agreement. B. Through its superintendent, approve staff access to the K-12 Data Mart on the MDE Secured Reports website in a manner consistent with governing data practices laws, including FERPA and Minn. Stat. § 13.32. FERPA requires that a local educational agency must determine that school officials, including teachers, have a legitimate educational interest before those officials have access to a student’s educational records. The superintendent will approve staff access on a case-by-case basis for each individual staff person who will have access to the K-12 Data Mart. By approving each individual staff person for access to the K-12 Data Mart, School District and its superintendent assure that they have made a determination that each staff person to be granted access has a legitimate educational interest. C. Notify MDE in a timely manner when a staff person’s access to the K-12 Data Mart should be revoked because they no longer have a legitimate educational interest for any reason, including but not limited to change of job responsibilities or departure or termination from school district employment. This notification requirement is satisfied by notifying the Minnesota Department of Education through established Secured Report protocols. D. Not give others within or outside the district access to the K-12 Data Mart or individual-level information retrieved via the K-12 Data Mart. Summary data and summary analyses created from SLEDS data that do not potentially identify individual students can be used and shared. E. Understand and comply with all of the Data Practices Provisions of this Agreement.

It is the responsibility of the School District to ensure that the buses are at the agreed upon location(s) at the agreed time(s); B.
