The questionnaire shall contain a series of questions that will be answered on a computerized answer key and rated on a standardized scale. The questionnaire will comprise approximately 20 core questions plus any optional questions, selected from an approved list, added by the employee being evaluated. The format of the questionnaire, the core questions, and the list of optional questions shall be mutually agreed to by the parties and shall not subsequently be changed without the mutual agreement of both parties.
2.2 With each questionnaire a separate sheet of paper shall be provided for students to make written comments in response any open-ended questions approved by the parties. These comments shall be returned unexamined to the employee being evaluated in accordance with 2.5.
2.3 Where possible, student questionnaires shall be conducted in one semester of the academic year for each faculty member in every course with an enrolment of five (5) or more students.
2.4 The student questionnaire shall be administered by clerical staff under the direction of the Xxxx or Director. Employees shall not receive any information that would disclose the identity of students who completed the questionnaire.
2.5 After questionnaires have been completed, the written comments shall be placed in a sealed envelope, which shall be held in the office of the Xxxx or Director and provided to the employee being evaluated, unopened, after the final date for grade appeals has elapsed.

(iv) The Employer shall make support services available to the Committee to facilitate its tasks.
(v) The teaching evaluation form shall consist of at least two sections. The results of Section I shall be aggregated, and shall be placed in the employee’s Official Personal File. (b) The instrument used, and the information required shall be determined by the Employer.
(c) In accordance with Article 8.3, any change to teaching evaluation instruments or teaching evaluation processes or both contemplated by Carleton University which may impact CUPE 4600 (Unit 2) employees will be brought to the JCAA for meaningful consultation with the Union prior to being implemented.
(a) The teaching evaluation score in any course shall be determined by the Instructor score reported in column B on the Retrospective Teaching Evaluation Report produced by the Office of Institutional Research and Planning (OIRP), or the Instructor score reported in the Average column on the Retrospective Teaching Evaluation Report, whichever is higher. The response rate in any course shall be determined by dividing the number reported in the “number of respondents” (# Resp) column by the number reported in the “enrolment” (Enrl.) column on the Retrospective Teaching Evaluation Report produced by the OIRP. Where the response rate in any course falls below fifteen percent (15%), the teaching evaluation score for that course shall be deemed anomalous and no further action will be required.

Teaching Evaluations a) Faculty Members’ teaching activities and output will be set out in their annual workplans.
b) In order to improve course design and teaching effectiveness, all courses taught will conduct a written evaluation of each Faculty Member’s teaching performance, provided that the students consent to participate in the survey, and provided that the procedures of the survey protect student confidentiality.
c) Evaluations administered to assess a Faculty Member's teaching performance shall be in accordance with the requirements below and with other provisions of this Agreement.
d) Student evaluation of teaching shall be obtained through a questionnaire administered by the University in such a way as to afford all the students in a given course or class a reasonable chance to respond. A common evaluation instrument will be administered across the University to ensure equity, consistency and fairness. Such an instrument shall be constructed so as to obtain a fair and reasonable assessment of teaching effectiveness, course design and course quality.
e) The design of the evaluation instrument will involve the creation of a committee including members of CTET, the Association, and University Administration. The existing teaching evaluation framework will be in force until the new evaluation protocols are agreed to.
f) Evaluations of courses will be conducted online. The evaluation instrument shall be made available online within the final week (in distance learning) or the final days (in residency) of any course. No questionnaire shall contain any indication of the identity of the student filling it out.
g) Both quantitative and qualitative results of the questionnaire will be used in evaluating the Faculty Member’s performance. All responses to each question shall be aggregated, and the mean, frequency distribution, and number of eligible respondents shall be calculated. The written student responses shall be as important as the numerical responses. The student’s numerical and written responses will form part of the Faculty Member's OPF. A copy of these aggregated data, along with the qualitative comments, shall be sent to the Faculty Member, School Director, and Xxxx.

Teaching Evaluations. (a) The primary goal in the use of student feedback, as part of an evaluation of teaching process, is to identify where the student learning experience can be improved, provide summary evidence of levels of student satisfaction, provide feedback on course content and delivery, and provide another means for recognising examples of good practice in teaching.
(b) Evaluation of teaching performance must be considered in the context of the teaching and learning environment and must take into account such issues as the size and prior knowledge of the class, the physical environment in which teaching takes place, the workload of the employee, the structure of the course and the provision of support and resources to the employee.
(c) Academic staff will seek evaluative student feedback on their teaching in at least one course each year using teaching forms approved by either the University’s Academic Board or by a Faculty Board.
(d) Once each year, an employee will provide to his/her academic supervisor a summary of teaching evaluations from at least one course agreed with the academic supervisor for which they have had some responsibility for teaching. The summary will clearly identify:
(i) the nature of the employee’s contribution to teaching for that particular course;
(ii) the number of students taught in the course;
(iii) a summary of the ratings provided by students on the items contained in the feedback questionnaire (there will be no requirement to forward any information relating to open-ended student comments).
(e) An employee will meet with his/her Head of School on an annual basis as part of the Career Development Scheme to discuss teaching related matters. This meeting will have a clear developmental and recognition aim. It will provide an opportunity for the Head of School to more formally recognise and commend excellent teaching, and where necessary, discuss and agree staff development activities to be undertaken to enhance or improve teaching quality.
(f) Feedback from students in the form of teaching evaluations will not be used by the University to initiate any action under clause 21.0 of this Agreement or as part of the criteria for selecting redundancies. Student feedback on its own cannot be used as a measure of teaching performance.
(g) Course evaluation data will not be published on the University website in a form that allows the identification of individual staff members.
