HEALTH & WELFARE PLAN. Employers and/or individuals who manage, operate, assist or own, either partially or wholly, a company or companies working non-union in the construction industry on Mainland Nova Scotia within the craft jurisdiction of xxx Xxxxxxxxxx Local 83 shall not be eligible to be appointed to serve, or to continue to serve, as trustees on any trust fund referred to within this Collective Agreement. This provision shall apply to management trustees and union trustees alike. The parties hereto agree on a Health and Welfare Fund as follows:
HEALTH & WELFARE PLAN. 8.01 a) On the first day of the calendar month following the completion of three (3) months of employment, employees who are actively working become eligible for the following benefits:
HEALTH & WELFARE PLAN. The Company will provide adequate Hospitalization, Medical-Surgical Insurance, Major Medical, Dental, Vision coverage, Life insurance, and supplemental insurance plans for associates covered by this Agreement in accordance with the plans and related terms that cover non-bargaining unit associates at the Kirksville, MO location, including, without limitation, plan design, open enrollment time frames, associate contributions, waiting periods, and the like. For the term of the agreement, employees will not be required to pay more than 45% of the cost of the PPO medical plan (or similar option), and no more than 35% of the cost of the HRA and Select medical plans (or similar options). In the event the Company is considering changes in group insurance coverage, the Company shall meet and discuss such changes with the union committee prior to any such change being implemented.
HEALTH & WELFARE PLAN. The Company will make monthly contributions to the Alaska Electrical Health & Welfare Trust (“H&W Trust”) on behalf of all regular employees covered under this Agreement, in accordance with the terms of the Plan.
HEALTH & WELFARE PLAN. 16.1 In order to protect all employees and their families from the financial hazards of accidents or illness, the Employer agrees to contribute two dollars and eighty‐one cents ($2.81) per hour earned by each employee, to the UA administered Health and Welfare Trust Fund, which will provide benefits in accordance with the Refrigeration Workers Health & Welfare Plan.
HEALTH & WELFARE PLAN. A Health & Welfare Plan has been implemented on a cost shared basis (with the exception of Group Health which shall be 65% Employer paid and 35% Employee paid). The Employer will cost share, as currently provided, the premiums for the existing (or similar coverage) dental plan in effect on the basis of 50% of the premiums payable for those Nurses who are eligible and who pay their respective share of 50% of the premiums payable.
HEALTH & WELFARE PLAN. (a) The Employer acknowledges the OPCMIA. Health & Welfare Plan of Alberta as established by Trust Agreement dated the 5th of April A.D. 1966 between the Edmonton General Contractors Association and the Operative Plasterers’ and Cement Masons’ International Association, Local 924, Edmonton, which agreement was amended the 30th day of April A.D. 1969 and became the Trust Agreement between the Edmonton General Contractors Association and the Calgary General Contractors Association and Independent Contractors of the first part, and the Operative Plasterers’ and Cement Masons’ International Association, Local 924, Edmonton, Alberta and the Operative Plasterers’ and Cement Masons’ International Association, Local 139, Calgary, Alberta of the second part together with all amendments thereto and agrees to be bound by the terms of that agreement, as amended from time to time by the decisions of the Trustees appointed from time to time thereunder.
HEALTH & WELFARE PLAN. 26.01 Eligible employees covered by this Collective Agreement will participate in the Company's health benefits plan program in accordance with the terms and conditions of the plan.
HEALTH & WELFARE PLAN. 29.01 The Employer shall pay 100% of the monthly premiums for the Medical Services Plan (M.S.P.)
HEALTH & WELFARE PLAN. 8.1 Employees with at least five (5) months seniority and that have already qualified for benefits shall be eligible for the following Employer paid benefits: Dental Extended Health Care Life Insurance AD&D Short Term Disability Long Term Disability Employees hired after April 26th, 2016 and those employees with less than six (6) months seniority that have not yet qualified for benefits shall be eligible for the New Employee Benefits Plan that shall be paid by the Employer: Dental Extended Health Care Life Insurance AD&D Employee contributions shall commence effective May 1st, 2016 to fund Short Term Disability and Long Term Disability benefits for employees covered by the New Employee Benefit Plan.
