Health Benefit Program. A Comprehensive Medical Plan designed to pay the major share of covered hospital, surgical and medical expenses, while attempting to control health care costs by encouraging the use of cost- effective services. A Vision Care Plan with no deductible, which includes an eye examination once every twelve months, one pair of lenses once every twelve months, and one pair of frames once every twenty- four months. Health Benefit Program. The Plan’s applicable program that defines the health care services for which a Plan Member is eligible and the conditions and circumstances under which the Plan is obligated to pay for such services on behalf of the Plan Member.
Health Benefit Program. The County will provide a Health Benefit Program which includes the following coverages: A. A Hospitalization/Surgical Medical Benefit Plan subject to a One Hundred ($100.00) Dollar co-payment which applies to each hospital admission up to a maximum of two (2) hospital admissions per calendar year per family. B. An unlimited maximum on Major Medical coverage after an initial $200.00 individual deductible/$400.00 aggregate deductible with 80% co-insurance up to $2,500.00. C. An eye care coverage plan for all Employees and their dependents covered under this Agreement. D. A Prescription Insurance Plan for all Employees and their dependents with an $8.00 co-pay for non-formulary or non-generic drugs. A $4.00 co-pay for formulary drugs or an alternate zero (0) co-pay for generic drugs. Effective July 1, 2005, the co-pays will increase to $10.00 for non-formulary and $6 for formulary. Effective January 1, 2007, the co-pays will increase to $12 for non-formulary and $8 for formulary. Employees may order prescriptions by mail where the prescriptions are repetitive maintenance medications which have been taken by the Employee for sixty (60) days and have been designated as repetitive maintenance medications by the County. Mail order prescriptions shall include a $12.00 co-pay with a $6.00 alternate co-pay for formulary drugs or an alternate zero (0) co- pay for generic drugs for a ninety (90) day supply. Effective July 1, 2005, these co-pays shall increase to $14.00 and $8.00. Effective January 1, 2007 the co-pays shall increase to $16.00 and $10.00. E. A disability coverage insurance plan with benefits of $150.00 per week for a period of twenty-six weeks (26) will be provided. Coverage under this program will begin when the Employee exhausts all accrued sick time and/or has completed an eight (8) scheduled workday waiting period. Employees will only be entitled to this $150.00 amount for an aggregate total of twenty-six (26) weeks for the life of this Agreement. Thereafter, they will only be entitled to the $90.00 per week for thirteen (13) weeks. Benefits available to Employees under this plan shall be denied and shall at any time be denied or discontinued by the County in any of the following events: 1. It finds said Employee unreasonably refuses, prevents or hinders medical examinations from time-to-time as the County may require. 2. Employee is found not to be disabled as herein provided. 3. Health Benefit Program. A. The Authority/CMCMUA shall provide medical coverage at not less than such coverages as the State of New Jersey may provide its employees, via the New Jersey State Health Benefits Program (NJSHBP) NJ DIRECT10-#050(1) Plan. The Authority/CMCMUA shall provide a prescription plan at no less coverage than the State of New Jersey provides to its employees, via the PRESCRIPTION DRUG PROGRAM #201 Plan. The Authority/CMCMUA shall provide a dental plan at no less coverage than the State of New Jersey makes available to its employees. The Authority/CMCMUA shall provide a vision plan equal to the plan in place as of January 1, 2013 as provided by UnitedHealthcare Vision (Spectera). B. The Authority/CMCMUA, however, reserves the right to review and change health benefit insurance coverages during this Contract as long as the level of coverage provided is comparable. C. Employees who have health benefits coverage through other sources (proof of other coverage required) may waive their health benefits offered through the Authority/CMCMUA and receive $2,500 per annum, payable on or about December 1st of each year, on a prorated basis. The payment will be prorated, as applicable, the first year the employee opts out of health benefits coverage. Under Chapter 2, P.L. 2010, multiple coverage in the NJSHBP or the New Jersey School Employees’ Health Benefit Program (NJSEHBP) is prohibited. Payment to employees for waiving health is only permitted if other coverage is through a non-NJSHBP/NJSEHBP plan. D. In the event a husband and wife or partners in a civil union are both employed by the Authority/CMCMUA, only one (1) designated spouse/partner shall be afforded primary health benefits coverage, with the other spouse/partner covered under the health benefit plan as a family member. Under Chapter 2, P.L. 2010, the non-designated spouse/partner is prohibited from receiving payment for waiver of coverage. E. Bargaining unit employees who are fifty-five (55) years of age, or older, and retire from the Public Employee's Retirement System (PERS) with twenty-five (25) or more years of service with the Authority/CMCMUA shall be eligible to receive Authority-paid health insurance benefits upon retirement from the Authority/CMCMUA as referenced in Authority/CMCMUA Resolution No. 98-98, dated September 2, 1998 and subsequent amendments thereafter. F. Employee contributions, by payroll deduction, to the premium costs of medical and prescription coverage, will be in accordance with the ...
Health Benefit Program. A program which defines and limits the medical and Facility Services for which a Member is eligible, includes the conditions and circumstances under which the Health Plan is obligated to pay participating providers for Covered Services on behalf of Members and defines the Copayment Charges for which a Member is responsible. The Health Benefit Programs are described in the Product Attachments.
Health Benefit Program. A. The Authority/CMCMUA shall continue to provide health coverage at not less than such coverages as the County of Cape May may provide to its employees. B. The Authority/CMCMUA, however, reserves the right to review and change health benefit insurance coverages during this Contract as long as the level of coverage provided is comparable. C. On or about December 1, of each calendar year, the Authority/CMCMUA shall inform employees of its basic standard health benefit plan and any alternate choice(s) to be made available to employees during the following calendar year. If the employee selects to be covered under the basic standard health benefit plan, then such plan will be provided to the employee and his/her dependents without charge. In the event an employee selects any alternate choice plan that is more expensive than the basic standard health benefit plan, then the employee shall pay a maximum of $20.00 per month for individual-only coverage and a maximum of $30.00 per month for individual/dependent coverage. Payment for such contributions shall be by way of biweekly payroll deduction and each employee must sign any necessary payroll authorization form in order to effectuate coverage under any alternate choice plan. Health Benefit Program. (A) All Employees covered under this Agreement shall continue to be covered by Horizon Blue Cross Blue Shield of New Jersey with the premium cost of the entire plan to be paid by the Employer.
