Sick Leave Procedure. Payments from cumulative sick leave credit shall be subject to the following conditions:
(a) A full-time employee shall, on the first day or part of a day of illness, report or cause to report such illness to the Immediate Non-Union Supervisor. Such reporting shall be in accordance with the rules governing reporting for work.
(b) A full-time employee who fails to report as outlined in subsection (a) of this Article 31.4 shall be considered as absent without leave and shall not be paid for this absence unless a reason is given that is acceptable to the Employer.
(c) A full-time or part-time employee shall file with the Immediate Non-Union Supervisor or designate, a doctor's certificate:
(i) after one (1) day of absence if deemed necessary and if so requested in advance in writing;
(ii) for each and every absence immediately following a vacation period;
(iii) for each and every absence on a day immediately prior to or after regular days off if deemed necessary, and if so advised in advance in writing.
(d) A full-time employee whose absence due to illness extends until after the third regularly scheduled working day shall, as soon as practical upon returning to work, submit a doctor's certificate;
(e) A full-time employee whose absence due to illness extends to fourteen (14) consecutive working days shall, on the fourteenth (14th) day and as requested thereafter submit the standard Haldimand County medical form, completed by a physician. The completed medical form may be submitted directly to the Coordinator, Benefit Services and the County will reimburse employees for any cost in obtaining such document, to a maximum of $40.00. In the event that the employer requires additional information outside of what is on the standard medical form, the employer shall pay a maximum of $75.00, unless otherwise deemed appropriate;
(f) A full-time employee failing to file a doctor's certificate or the standard Haldimand County form, shall be considered as being absent without leave and shall not be paid for this absence unless a reason is given that is acceptable to the Employer.

(a) An employee shall, on the first day or part of a day of illness, report or cause to report such illness to the Supervisor.
(b) An employee who fails to report as outlined in subsection (a) shall be considered as absent without leave and shall not be paid for this absence unless a reason is given that is acceptable to the Employer.
(c) When requested, an employee who is absent due to illness for three consecutive regularly scheduled working days or more shall, as soon as practical upon returning to work, submit a medical form that is completed by a physician;
(d) An employee who is absent due to illness for fourteen (14) consecutive calendar days or more shall submit a medical form that is completed by a physician;
(e) The Employer may request, in advance in writing, for the employee to submit a medical form completed by a physician after one day of absence if deemed necessary;
(f) An employee failing to file a medical form when requested as outlined above shall be considered as being absent without leave and shall not be paid for this absence unless a reason is given that is acceptable to the Employer.
(g) For the purpose of this Article, the medical form shall be provided by the Employer. The completed medical form may be submitted directly to the Coordinator, Benefits Services.
Sick Leave Procedure. In order for employees to receive sick leave compensation, the procedures below will be followed: The employee must notify the immediate supervisor or designee at the work location of his/her absence within the first hour of work of the first day of absence unless conditions make notification impossible. The employee who has been absent five days or more and who has been notified that a substitute has been hired shall, at least one day prior to his/her expected return to work, notify his/her supervisor so that the employment of any substitute employee may be terminated. An employee absent for five working days or more may be required to present a physician's statement stating the nature of the illness or injury and the date the employee is able to return to work. The District will require verification of all absences if there is reason to know or suspect abuse of the personal illness and injury leave by an employee. Any such request for verification shall be preceded by a written explanation and warning from the District that the employee will be required to provide such certificate prior to any further sick leave usage. Any such written warning shall remain in effect for twelve
Sick Leave Procedure. Faculty members who must be absent because of sick leave shall notify their administrator as soon as practicable under the circumstances; generally at least within one hour of reporting time. Faculty members shall file with their administrator a completed absence form within three (3) working days following the period of sick leave absence. The faculty member shall only be required to reflect on the absence form the actual number of contact hours, office hours, or other scheduled hours missed during the faculty member’s absence. A faculty member who misses one full week of work shall reflect forty (40) hours on the absence form. The administrator shall approve or deny the form and notify the faculty member. Disciplinary action may be taken if a faculty member fails to file or falsifies an absence report as provided in this section. Immediate family, for the purpose of this section, includes: parent, sibling, grandparent, child, spouse, parents-in-law, children-in-law, sibling-in-law, grandchild, stepparents, stepchildren, a legal guardian or other person who stands in the place of a parent, other persons residing in the home of the faculty member, or a domestic partner. To use sick leave for the death, illness, or injury of a domestic partner, a completed and notarized Affidavit of Domestic Partnership form must be on file with the Human Resources Department. An authorized administrator may request information from a faculty member requesting sick leave to determine whether the sick leave request should be approved. A health care provider’s statement, if available, will be provided. If a faculty member’s sick leave meets the conditions of the College’s Family and Medical Leave Policy, the absence will also be counted as Family and Medical Leave. A faculty member who taught during three (3) of the past five (5) summer terms, or who has notified their department Chairperson by the time that summer teaching assignments are made in the department that they intend to teach summer term, shall be permitted to use Family and Medical Leave and available sick leave to cover short-term or long-term absences due to illness or injury occurring over all or part of a summer term. If a faculty member is taking Family and Medical Leave for a reason that meets the criteria for taking sick leave the faculty member shall, while on FML, utilize their accumulated sick leave balance. Sick Leave Procedure. A. Employees are required to report the need for sick leave in accordance with Department policy. Employees shall notify the scheduling system prior to the start of their scheduled duty shift when utilizing sick leave. The Fire Chief or designee may waive this requirement based on exigent or unique circumstances.
B. If an employee is on vacation and a permissible use of sick leave occurs while on vacation, the employee may report the need for sick leave in accordance with Department policy and request to be taken off vacation leave and be placed on sick leave for the hours the employee is eligible for sick leave as specified in Section 14.2 of this MOU.
C. The Fire Chief may request a physician’s certificate or a personal affidavit verifying an employee’s need for sick leave when there is evidence of excessive sick leave usages and/or a pattern of sick leave misuse, abuse or falsification (i.e., using sick leave in conjunction with days off, vacation, holidays or otherwise).
D. Employees who are eligible for temporary disability payments may use accumulated sick leave in order to maintain their regular income; provided, however, the employee is allowed a credit towards sick leave to the extent that temporary disability payments are retained by the Department.
E. Any purposed changes to the Department’s current attendance management program shall be negotiated with the Union.
Sick Leave Procedure. 1. It shall be the employee's responsibility to notify their immediate supervisor at least thirty (30) minutes prior to the start of their work day with the reason for their absence.
2. Sick leave shall be awarded on an hour for hour basis, in quarter hour increments, for all employees.
3. Any employee requesting sick leave for reasons which are not consistent with this Article shall be denied leave time and may be subject to discipline for inappropriate use of sick leave as described in Section A-3 of the disciplinary guidelines of the Fire Rescue Department Manual.

(i) after any absence if deemed necessary and if so requested in advance in writing;
(ii) for each and every absence immediately following a vacation period;
(iii) for each and every absence on a day immediately prior to or after regular days off if deemed necessary, and if so advised in advance in writing.

Sick Leave Procedure. 12.1 In order to be eligible for sick leave with pay, an employee must:
a. Report promptly to the Fire Chief or designee the reason for his/her absence;
b. Keep the Fire Chief or designee informed on his/her condition if the absence is of more than three shifts duration;
c. Permit the District to make such medical examinations as it may deem desirable; and
d. Upon request of the Fire Chief or designee, furnish satisfactory evidence of incapacity upon return to duty from absences on sick leave of three (3) shifts or longer. A written report from a duly licensed and practicing physician, or other recognized practitioner stating the nature, duration and extent of each incapacity shall be deemed satisfactory evidence of incapacity. No salary or wages shall be paid for any period of absence under this section where the employee has failed to comply with the requirements of this subsection.
Sick Leave Procedure. Employees are required to attend work as scheduled. When unable to attend, the Employee must contact her/his immediate Manager, personally or by voice mail, as far in advance as possible, but no later than sixty (60) minutes to her/his scheduled starting time.
