Responsibilities of the School. 1. The SCHOOL will plan and determine the adequacy of the educational experience of the students in theoretical background, basic skill, professional ethics, attitude and behavior and shall assign to the HOST AGENCY only those students who have satisfactorily completed the prerequisite didactic portions of the SCHOOL’s curriculum. 2. The SCHOOL will retain ultimate responsibility for the education and assessment of its students. The School’s representative for this Agreement shall be a faculty member appointed and assigned by the SCHOOL, who will be responsible for medical student teaching and assessment provided pursuant to this Agreement. 3. The SCHOOL will advise all students assigned to the HOST AGENCY facilities regarding the confidentiality of patient/client records and patient/client information imparted during the training experience. The SCHOOL will also advise all students that the confidentiality requirements survive the termination or expiration of this AGREEMENT. 4. The SCHOOL will require all participating students to maintain health insurance and provide proof of health insurance to the School. The HOST AGENCY may request the student provide proof of health insurance prior to beginning of the training experience. 5. The SCHOOL will require all participating students to have completed an appropriate criminal background check, and to have documented appropriate immunizations on file with the SCHOOL. If applicable, the HOST AGENCY shall notify the student of any requests for evidence of criminal background test or immunization. The SCHOOL will inform the student of his/her responsibility to provide evidence to the HOST AGENCY of any required criminal background checks or immunizations, when requested. The HOST AGENCY shall notify the SCHOOL of its requirements of an acceptable criminal background check and required immunizations. The SCHOOL will also inform students that they may be required to undergo a drug test or other similar screening tests pursuant to the HOST AGENCY’S policies and practices, and that the cost of any such test will be paid by the student, if not the HOST AGENCY. 6. The SCHOOL will advise students that they are required to comply with HOST AGENCY rules, regulations, and procedures. 7. If requested by the HOST AGENCY, the SCHOOL will provide instruction to the HOST AGENCY'S staff with respect to the SCHOOL’s expectations regarding assessment of the SCHOOL'S students at the HOST AGENCY. 8. Responsibilities of the School. The school will provide a Catholic education for the student during the period of enrolment in accordance with its school curriculum, as amended by the school from time to time at its absolute discretion. Responsibilities of the School. 3.1 To hold public liability insurance for at least five million pounds which covers all reasonably foreseeable risks arising from the presence of Volunteers or other Number Champions representatives on the School’s premises in furtherance of the Agreement. On request, the School will provide a copy of insurance policies it holds to Number Champions. 3.2 To comply with all relevant legislation, codes and practices on Health and Safety, safeguarding, data protection, privacy, and equality in relation to the Engagement and the Volunteers. 3.3 To perform a DBS (Disclosure and Barring Service) check on the Volunteer before the Volunteer starts in the School, or otherwise to confirm that the Volunteer has a valid and current enhanced DBS certificate with child barring and to confirm the DBS number to Number Champions. 3.4 To provide the Volunteer with an appropriate induction to the School, including:- 3.4.1 the School’s Safeguarding Policy and relevant contact details, together with a practical overview of how to identify potential safeguarding issues with a child including identification of current issues relevant to the school community; and 3.4.2 the School’s Health and Safety Policy and all relevant associated information. This should cover, in particular, appropriate action in fire or other emergencies, and reporting procedures for accidents and unsafe situations. For the duration of the Covid 19 pandemic it should also cover arrangements to protect against infection. 3.5 To take reasonable steps to ensure that the Volunteer complies with School policies, including giving timely feedback if the Volunteer is seen to carry out any non-compliant action. 3.6 For each Child, including Children whom the School substitutes for other Children previously assigned, to procure that the teacher completes the following forms and to consolidate these forms and to return them to Number Champions in a timely manner:- 3.6.1 In September (or at the time the Child is assigned) the “Teacher’s selection of child” form; and 3.6.2 In June or the first week of July (or at the time the Child is removed from the Engagement) the “Evaluation of impact by the teacher” form. All fields of these forms shall be completed. In addition, the School shall confirm to Number Champions which Child is assigned to which Volunteer. Responsibilities of the School. 1.1 The School will keep confidential all documents, data, information, and other material provided to or obtained by either of the parties related to: service recipient care; Student performance; the particular mental health, physical, learning or environmental disabilities, challenges, or conditions, or personal circumstances of Students; and all other information identified by either party as confidential or personal information, in accordance with social work values and the CASW Code of Ethics, the Access to Information and Protection of Privacy Act (ATIPPA) and other applicable privacy legislation, the CYCP Act of Newfoundland and Labrador and successor legislation, and the terms of the Agency/School BSW Field Practicum Agreement and the BSW Field Practicum - Information Sharing Agreement. Students in the Field Setting will have access to patient/client personal information that is subject to strict confidentiality provisions under the CYCP Act and information obtained during field practica will only be shared with the School on a non-identifying or de- identified basis, unless permitted by the CYCP Act. Within the School, said information may be accessed by School-based Field Instructors, Field Education Coordinators, BSW Student Services Coordinator and the Field Secretary. All equipment purchased becomes the responsibility of the School. viii. announce the Grant through the School’s communication vehicles including (but not limited to) newsletters, intranet and website, subject to obtaining the consents required by Section 3 (Promotions) of the Terms and Conditions. ix. co-ordinate all correspondence between Best Buy and the School’s departments and staff through the School’s business development department; x. ensure that any individuals involved in the promotion of the Grant have signed the requisite waivers and releases provided by Best Buy; xi. if requested by Best Buy, help support a public relations event at the School to announce or celebrate the Grant (the “PR Event”). The PR Event will occur during school hours. Media and dignitaries will be invited to attend the PR Event if appropriate; and xii. send Best Buy a report no later than six months after the Deadline Usage Date including a summary of how the Grant has impacted the student experience at the School, and sharing any anecdotal observations/photos about the impact the Grant has provided. Reporting can be electronic or hard copy.
