Voluntary Sick Leave Bank. Purpose: The purpose of the Voluntary Sick Leave Bank is to provide personal illness leave to contributors to the Bank after their accumulated compensatory leave has been exhausted, and more specifically, to provide such leave from the Bank in case of prolonged illnesses.
Voluntary Sick Leave Bank. The Association shall be permitted to establish a voluntary sick leave bank. Rules and regulations for the governing of the affairs of this voluntary sick leave bank shall be as established by the Association, provided, however, that certain minimum requirements shall be included in such rules and regulations which shall provide that a teacher must have used all accumulative sick leave days before application can be made to the bank (provided, however, a teacher shall not be allowed to use sick leave bank days when such teacher is eligible to qualify for payment under the School Corporation’s long-term disability insurance program) that the teacher must have been sick five (5) consecutive days or ten (10) non-consecutive days beyond the amount of current sick leave during one (1) contract year and that the teacher must furnish a doctor’s certificate as proof of illness or injury. The conduct of the affairs of the voluntary sick leave bank shall be under a committee or board appointed by the Association. When possible, the committee shall advise the Board of its meeting dates, and if desired by the Board, a representative of the Board may sit in on meetings of the committee as a non-voting member. Upon retirement, a teacher may elect to donate up to 10 unused sick leave days if they are in excess to the number of days that were bought out as listed in Attachment #1. The maximum number of days allowed in the Sick Leave Bank at any time is 500.
Voluntary Sick Leave Bank. A Voluntary Sick Leave Bank will be established and maintained by the voluntary contributions of accrued sick leave days by full-time contract and regular faculty. The intent of the Voluntary Sick Leave Bank shall be to assist and aid contract and regular faculty who experience unusual circumstances that cause a need for sick leave beyond that which they have earned and accrued. The Voluntary Sick Leave Bank shall be managed by the District's Human Resources Office.
Voluntary Sick Leave Bank. Any teacher may contribute to the sick leave bank program during the month of October of each school year by contributing one (1) sick leave day of the current year’s sick leave to the sick leave bank on a form prescribed by the Association President. The Sick Leave Bank Council (SLBC) shall review and determine the number of days which may be granted. The SLBC shall consist of four (4) members as follows: the President of the OTA, one other OTA member, the Superintendent or designee and the Chief Operations Officer. The decision of the SLBC is final and binding and is not grievable under Article 9 of this contract. The procedures of the sick leave bank shall be as follows:
1. All teachers may apply for use of additional sick leave days by application to the SLBC by submission of application to the Superintendent or designee.
a. The SLBC shall be responsible for developing any forms that may be required and for keeping all necessary records.
b. The SLBC shall formulate any regulations deemed necessary for the operation of this program.
c. The SLBC shall notify the District Treasurer of the sick leave bank awards. The District Treasurer shall be responsible for the accounting of the sick leave bank days.
2. Teachers may apply for additional sick leave days from the sick leave bank after the following conditions have been met:
a. Their own accumulated sick leave days have been exhausted, their advanced five (5) days of sick leave have been used, and they are not eligible for disability leave under State Teachers Retirement System and/or Workers’ Compensation;
b. The teacher shall submit in writing to the SLBC, the reasons for the request of additional sick leave bank days along with attending physician’s statement pertinent to the member’s request.
3. Only earned sick leave may be contributed to the bank. Advanced sick leave shall not be contributed to the bank.
4. The maximum number of days that can be granted to any teacher is thirty
Voluntary Sick Leave Bank. 1. The purpose of the Voluntary Sick Leave Bank is to provide extended sick leave coverage to any employee benefited by this agreement who has exhausted his/her sick leave and who is a member of the Voluntary Sick Leave Bank pursuant to Paragraph B. below. An employee may access the Voluntary Sick Leave Bank for absences due to or resulting from an extended and/or serious injury, illness or disability as defined by the Family and Medical leave Act as a serious personal illness or injury.
2. To be eligible to join the Voluntary Sick Leave Bank as a member, an employee must have completed one (1) year of employment and must contribute one (1) day of his/her accumulated sick leave to the Bank.
3. The Bank will be administered by a committee of four persons, two members representing the Department and two members representing the Association, to be known as the Sick Leave Bank Committee.
4. All requests for the use of sick days from the Voluntary Sick Leave Bank shall be directed to the Sick Leave Bank Committee. The Sick Leave Bank Committee may approve a request for the use of Voluntary Sick Leave Bank days in an amount up to ninety (90) days. The total grant of Voluntary Sick Leave Bank days shall not exceed 1 (one) year per illness, injury or disability. A request for the use of Voluntary Sick Leave Bank Days must be accompanied by an application and medical documentation. Any requests for an extension of Voluntary Sick Leave Bank benefits of more than ninety (90) days, must be accompanied by a new application and current medical documentation.
5. In acting upon requests for sick days from the Voluntary Sick Leave Bank, the Sick Leave Bank Committee shall utilize the following criteria:
A. adequate medical evidence;
B. Prior utilization of sick leave and/or Voluntary Sick Leave Bank
C. reason for and propriety of prior use of sick leave; and
D. the member’s intent to return to service in the Ayer Police Department
6. In order for a member to be eligible for Voluntary Sick Leave Bank benefits in each fiscal year, he/she must return to work for a period of time at lease as long as the period for which he/she received benefits. The Sick Leave Bank Committee may waive this provision in extraordinary circumstances.
7. The decisions of the Sick Leave Bank Committee are final and not subject to the Grievance and Arbitration Procedure.
8. Implementation open.
Voluntary Sick Leave Bank. A voluntary sick leave bank shall be available. The Association and Board shall agree to the structure and guidelines of the bank as stated in Appendix A.
Voluntary Sick Leave Bank. Any employee covered under the terms of this contract shall be eligible to participate in a VOLUNTARY sick leave bank. The intent is to provide extended sick leave benefits to those persons who incur a period of extended illness, injury, or hospitalization. A person will only be eligible for sick leave bank benefits after using all personally accumulated sick days, personal days, and subsequently one (1) day of pay deduction. New participants in the voluntary sick leave bank shall submit written notice by August 30 of intent to participate on a form provided by the United Education Association. If a new employee is hired during the school year he/she may take part in the sick bank by submitting intent to participate form provided by the UEA. They will have two (2) weeks from the first day of employment to decide if he/she wants to take part in the sick bank. Each employee electing to participate in the bank shall contribute one (1) sick day to the sick bank each year until the sick leave bank reaches the minimum capacity of one hundred (100) days. In case of depletion of the sick leave bank below one hundred (100) days all employees enrolled will automatically have one (1) sick day deducted and added to the sick bank. The UEA president will notify all members of the sick bank in writing of the depletion of sick bank days and that an automatic one (1) sick day will be taken out of their personal sick days on said date that it will occur. Membership is automatically renewed each year unless a member submits written notice of cancellation by August 30 of the school year in which cancellation is desired. A participant may cancel membership by indicating in writing directed to the attention of the UEA president. Cancellation of sick leave bank membership, regardless of reason, shall mean forfeiture of any claim to their contributed days and benefits of membership. The intent of the bank is to provide additional financial protection to those employees who incur a period of prolonged illness, injury, or hospitalization, as verified in writing by a physician. The bank is not applicable to any employee utilizing days for illness in the family except in emergency situations following approval of the Superintendent and UEA president. It remains the intent of both parties to strive to retain good attendance in the district. Voluntary Sick Leave Bank. Lengthy illness, or injury, may exhaust an employee’s sick leave and vacation leave, creating a hardship for the employee. To provide additional assistance to BED employees, a volunteer sick leave pool has been established to which employees may donate vacation leave eligibility to benefit fellow employees. The sick leave pool is designed, and has been implemented, primarily to assist employees who have not yet been able to accumulate sick leave or who may encounter a serious illness or injury. Leave Donation: Any employee may donate vacation leave time to the Pool, but must have at least one (1) week of vacation time remaining after any donation. Donations may be made any time throughout the year, and must be made in blocks of eight (8) hours. The sick leave pool will accumulate from year to year. The identity of the donors and recipients shall remain confidential. Eligibility: To be eligible to receive sick leave from the pool, an employee must have exhausted all his/her accumulated sick leave, have exhausted all but one (1) week of their vacation leave, and have a reasonable expectation of returning to work within six (6) months of his/her last paid and/or vacation leave day. Procedure: An employee must request that he/she receive days from the sick leave pool. Requests will be reviewed by a review committee made up of one non-union employee appointed by BED management and one union employee appointed by the IBEW Business Manager. Awards will normally be made in blocks of five (5) normal working days. No sick leave or vacation accruals shall occur for individuals who are utilizing the sick leave pool.
Voluntary Sick Leave Bank. 54 The purpose of the Voluntary Sick Leave Bank (hereafter referred to as the Bank) is to 55 provide sick leave to contributors to the Bank after their accumulated sick leave has been 2 illness. The Bank rules and guidelines are as follows:
