Health and Other Insurance. The District shall pay the premiums for hospitalization, major medical, dental, vision and basic life insurance coverage for the Superintendent to the same extent such premiums are paid by the District for its executive administrative employees on 12-month contracts. The District shall also make available to the Superintendent other benefit programs such as flexible spending accounts, 403(b) plans, disability plans and other programs to the extent those programs are made available to District executive administrative employees on 12-month contracts. The District shall add the premium for any of the benefits listed in this Section 3.5 to the taxable income of the Superintendent if (a) the District is required to do so to comply with federal tax and/or employee benefits laws or (b) so requested by the Superintendent in order to make the payment of any benefits to him more advantageous under federal tax and/or employee benefits laws.

Health and Other Insurance. City shall provide Xxxxx with the same long term disability, medical, dental, life, and vision insurance coverage and benefits as are provided other management employees of the City, except as follows: City shall pay the employee's (family plan) premium for health, vision and dental insurance from available options though its insurance carrier. Xxxxx shall be responsible to pay any and all deductibles, co-payments, or other charges above the health insurance premium.

Health and Other Insurance. Employee (which term in this context under this provision shall include his dependents) shall be entitled to participate in whatever group medical, dental, life insurance and pension benefit plans are offered by or through the Town of Rockland on the same basis as other Town employees. The Employee shall be eligible for up to Two Thousand, Five Hundred Dollars ($2,500) per year reimbursement for an insurance policy. It shall be the sole responsibility of the Employee to purchase and maintain said policy. The Employee shall annually submit verification of the insurance policy to be eligible for reimbursement.
Health and Other Insurance. The City Manager may participate in any insurance plan or plans, if any, offered from time-to-time by the City to non-civil service City employees. Such participation by the City Manager and any corresponding payment of premium(s) shall be on the same terms and conditions as available from time-to-time to other non-civil service employees. Participation in any insurance plan or plans by the City Manager on or after the date of termination or resignation shall be on the same terms and conditions as continuing participation is available, if at all, to other non-civil service employees on the date of the City Manager's termination or resignation.
Health and Other Insurance. The Superintendent shall be entitled to and the Agency shall provide comprehensive family group health and hospitalization, dental and vision insurance and prescription drug insurance coverage applicable to such twelve (12)-month non-bargaining employees as are incident to their employment relationship with the Service Agency and as referenced in the 2019-2020 Benefits Enrollment Guide. The Service Agency contribution for medical benefits shall be limited in accordance with Section 3 ofPA 152 of2011 (Publicly Funded Health Insurance Contribution Act). The Superintendent shall be entitled to term life insurance policy in an amount equal to three (3) times his annual salary.

Health and Other Insurance. The District shall contribute the sum of $1,208 per month to the Superintendent’s flexible benefit account effective July 1, 2019 which may be used for payment of insurance premiums. The Superintendent must participate in the District’s long term disability and the District shall provide life insurance at its own expense, which will be three times the annual salary of the Superintendent up to a maximum of $420,000 in life insurance. The Superintendent may opt to purchase additional insurance from the District carriers if allowed by the carrier. The Superintendent is responsible for the cost of monthly insurance premiums that exceeds the amount of the School District’s monthly contribution to her flexible benefit account, and such amount shall be paid by payroll deduction.
Health and Other Insurance. Subject to the rules of any applicable scheme, the Company shall continue to cover the costs of membership of the Employee and his dependants of a private medical insurance scheme with BUPA or such other reputable medical expenses insurance scheme as the Company shall decide from time to time through December 31, 2013. The level of insurance cover to be provided shall be the same as the level of insurance cover provided by the Company immediately prior to the Termination Date. In addition, the Company will continue to meet the Executive’s compulsory annual contribution to TEBE or any equivalent state insurance or pension fund at the same level as provided immediately prior to the Termination Date, through December 31, 2013. The Company shall continue to provide the Employee with protection against loss of life and against personal accident under a policy of insurance, with sums insured at the same level as was provided by the Company immediately prior to the Termination Date.
