Trainee Responsibilities. As a condition of Appointment and of ongoing participation in the Program, Trainee must meet the prerequisites and qualifications for trainee eligibility as outlined in the Accreditation Council for Graduate Medical Education (ACGME) Institutional Requirements (xxx.xxxxx.xxx), or as set forth in such other applicable accreditation requirements, or requirements the Hospital promulgates and follows as applicable to non- ACGME accredited programs, that may be in effect from time to time. In providing services and in participation in the activities of the Program, Xxxxxxx agrees to do the following: 2.1. Xxxx and adhere to the Medical Staff Bylaws, the policies and practices of DUHS, Hospital, and Program (accessible at https.//xxx.xxxx.xxx/xxxxxxxx), the policies and requirements of all of the facilities to which Trainee is assigned for clinical rotations, the standards of all accrediting certifying or licensing organization, and all applicable state, federal and local laws, including all public health requirements or orders; 2.2. Participate fully in the educational, research and scholarly activities as assigned by the Institution and Program (and/or as necessary for the completion of applicable graduate requirements), attend all required educational conferences, assume responsibility for teaching and supervising other trainees and students, and participate in assigned DUHS, Hospital, Medical Staff committee activities; 2.3. Use their best efforts to provide safe, effective and compassionate patient care, and present at all times a courteous and respectful attitude toward all patients, colleagues, employees, visitors at DUHS and other facilities/rotation sites to which Trainee is assigned. This provision includes compliance with the dress code applicable to DUHS trainees; 2.4. Satisfactorily fulfill the educational requirements of the Program in a timely manner; 2.5. Provide clinical services: 2.5.1. As and when assigned 2.5.2. Commensurate with their level of advancement and responsibilities; 2.5.3. Under appropriate supervision; 2.5.4. At sites specifically approved by the Program and Office of Graduate Medical Education; and 2.5.5. Obey and adhere to the Medical Staff Bylaws and Corrective Action and Hearing Procedures for Residents and policies and practices of the department(s) and Hospital, including but not limited to the following: 2.1.1. Obey and adhere to the corresponding policies of all of the facilities to which he/she rotates, including completion of all facility required education programs. 2.1.2. Obey and adhere to all applicable state, federal, and local laws, as well as the standards required to maintain accreditation by The Joint Commission (JCAHO), the ACGME, the American Osteopathic Association (AOA), the Residency Review Committee (RRC); and any other relevant accrediting, certifying, or licensing organization, including the State of Michigan Medical Board. 2.2. Participate fully in the educational and scholarly activities of the program, including the performance of scholarly and research activities as assigned by the Institution (Orientation and Core Curriculum) Program Director (and/or as necessary for the completion of applicable graduate requirements), attend all required educational conferences, assume responsibility for teaching and supervising other Physician Trainees and students, and participate in assigned WSU, Hospital, and medical staff committee activities. 2.3. Use his or her best efforts to provide safe, effective, and compassionate patient care and present at all times a courteous and respectful attitude toward all patients, colleagues, employees, visitors at WSU and other facilities/rotation sites to which the Physician Trainee is assigned.
Trainee Responsibilities. Upon being served with a subpoena, a trainee shall immediately review the training schedule, and identify the Scheduled FTO. The trainee shall make a copy of the subpoena and place it in the Scheduled FTO’s mailbox. The trainee shall also leave a message in the Scheduled FTO’s voicemail, which includes details of the subpoena and the date and time of appearance.

Trainee Responsibilities. A. Trainee understands that this Agreement is contingent upon: 1. Verifying identity and eligibility to work in the United States and to complete the required employment eligibility verification form upon hire, in compliance with federal law, and to maintain such eligibility; 2. Taking and passing the pre-employment physical exam administered by Beaumont, which includes laboratory tests to detect substance abuse and nicotine; 3. Obtaining and maintaining a valid State of Michigan medical license and controlled substance license, as required; 4. Providing true and accurate information; and 5. Not being convicted of a felony. If the employability, drug screen, or license requirement is not fulfilled or maintained, or if there is a misrepresentation, material omission or falsification of any information supplied to Beaumont by the Trainee as part of the entrance requirements of the Program, or if Trainee is convicted of a felony, this Agreement may be canceled, terminated or effective date delayed without Trainee pay, by Beaumont, without provision of the due process otherwise provided herein. B. Trainee specifically agrees: 1. To fully participate in Program activities and to comply with all Program requirements. Trainee shall serve responsibly during the entire period agreed upon as specified in this Agreement. 2. To observe all Program Policies, GME Policies and Beaumont Policies, and to fulfill all educational and professional responsibilities as determined by the Program Director, including those pertinent to work (duty) hour compliance and documentation of such. 3. To meet all financial obligations incurred to Beaumont, including, but not limited to, charging food items in dining areas through deductions from salary, if applicable. 4. To consider the salary, experience, and instruction received as sole compensation, and not to accept fees in any form from patients or physicians. 5. To submit to the Program Director in a timely manner all computerized or written evaluations of faculty, residents, medical students, educational experiences, and other evaluations as requested. 6. Trainee Responsibilities. 5.1 Trainees must attend all academic and work based sessions as required by their chosen course. Episodes of sickness and absence must be reported in accordance with Trust and policies. 5.2 Trainees are encouraged to have an honest, open dialogue with their line manager and advise them of any difficulties they face as early as possible. 5.3 If the trainee chooses to discontinue their training then their training contract will cease and be classed as a resignation from the trainee agreement.
Trainee Responsibilities. During the term of the Agreement, the Trainee shall perform the duties and satisfy the requirements described below. Trainee acknowledges that failure to fulfill any or all of these duties and requirements shall be considered a material breach of this Agreement and may lead to corrective action, including but not limited to termination, suspension, probation, or non-renewal of this Agreement. While performing his or her duties at the Hospital or at other locations as directed by the Hospital, the Trainee agrees to conduct him/herself in accordance with the Rules and Regulations of the Medical Staff; the Medical Education Policies, Procedures, and other governing documents; the directions of the Graduate Medical Education Committee, the Designated Institutional Official (“DIO”) or Director of Medical Education (“DME”), and the Program Director; the Policies and Procedures of the Hospital and OhioHealth; the Hospital’s House Staff Handbook, if applicable; OhioHealth employment policies; and the OhioHealth Code of Conduct. If this Agreement conflicts with any of the foregoing, this Agreement controls. Trainee must dress and carry out his/her duties in a professional and ethical manner in accordance with State and Federal laws and state licensure standards. B. If the Trainee has or becomes aware of any health condition that could have an effect on the Trainee’s ability to practice medicine, or if the performance of essential functions of his or her job requires reasonable accommodation in order to facilitate the Trainee’s continued practice of medicine or essential requirements of his or her job, the Trainee shall inform the Program Director and the DIO or DME, who together with OhioHealth’s Associate Health and Wellness Department shall determine what reasonable accommodations, if any, shall be made. Such conditions include but are not limited to, substance abuse, mental illness, physical disability, or contagious diseases such as HIV or tuberculosis. C. The Trainee must have met and completed all the requirements to receive his or her medical school degree. The degree must be conferred before the Trainee begins the Program. The Trainee must maintain the medical school degree throughout the term of this Agreement and throughout participation in the Program.
