Application Review Information. In order to receive a cooperative agreement under this FOA, applicants must submit an application, in the required format, no later than the deadline date. If an applicant does not submit all of the required documents and does not address each of the topics described below, the application will be disqualified. As indicated in Section IV. 2, Content and Form of Application Submission, all applicants must submit the following: 1. Cover Sheet 2. Standard Forms 3. Cover Letter 4. Project Abstract 5. Project Narrative 6. Work Plan and Timeline 7. Budget and Budget Narrative(s)
Application Review Information. 1. EVALUATION CRITERIA Section A: Statement of Need (15 points) 1. Document the need for an enhanced infrastructure to increase the capacity to implement, sustain, and improve effective medication-assisted treatment services that is consistent with the purpose of the program and intent of the FOA. 2. Describe the service gaps and other problems related to the need for infrastructure development. Identify the source of the data. Documentation of need may come from a variety of qualitative and quantitative sources. Examples of data sources for the quantitative data that could be used are local epidemiologic data, state data (e.g., from state needs assessments, SAMHSA’s National Survey on Drug Use and Health), and/or national data (e.g., from SAMHSA’s National Survey on Drug Use and Health or from National Center for Health Statistics/Centers for Disease Control and Prevention reports, and Census data). This list is not exhaustive; applicants may submit other valid data, as appropriate. Section B: Proposed Approach (35 points) 1. Describe the purpose of the proposed project, including a clear statement of its goals and objectives. These must relate to the performance measures you identify in Section D, Data Collection and Performance Measurement. Describe how achievement of goals will increase system capacity to support effective substance abuse and/or mental health services. 2. Describe the proposed project activities, how they meet your infrastructure needs, and how they relate to your goals and objectives. These should align with Section I-2, Expectations, of this FOA. 3. Provide a chart or graph depicting a realistic timeline for the entire three years of the project period showing dates, key activities, and responsible staff. These key activities should include the requirements outlined in Section 1-2: Expectations. [Note: The timeline should be part of the Project Narrative. It should not be placed in an attachment.] 4. Describe how key activities in your timeline will be implemented. 5. Describe how the proposed activities will adhere to the National Standards for Culturally and Linguistically Appropriate Services (CLAS) in Health and Health Care (go to xxxx://XxxxxXxxxxxxxXxxxxx.xxx.xxx). Application Review Information. ‌ 1. EVALUATION CRITERIA‌ The Project Narrative describes your plan for implementing the project. It includes the Evaluation Criteria in Sections A–D below. Your application will be reviewed and scored according to your response to the evaluation criteria. In developing the Project Narrative use these instructions: • The Project Narrative (Sections A–D) may be no longer than 10 pages. • You must use the four sections/headings listed below in developing your Project Narrative. Before the response to each criterion, you must indicate the section letter and number, i.e., “A.1”, “A.2”, etc. You do not need to type the full criterion in each section. • Do not combine two or more criteria or refer to another section of the Project Narrative in your response, such as indicating that the response for B.2 is in C.1. Reviewers will only consider information included in the appropriate numbered criterion. • Your application will be scored based on how well you address the criteria in each section. SECTION A: Population of Focus and Statement of Need (30 points – approximately 2 pages) 1. Identify and describe the geographic area where the project will be implemented and the youth (up to 24 years) that will be impacted. Document the need to increase your capacity to implement, sustain, and improve effective suicide prevention services in the proposed catchment area that is consistent with the purpose of this NOFO. Application Review Information. Eligible recipients are required to provide measures of effectiveness that will demonstrate the accomplishment of the various identified objectives of the CDC-RFA-TP17- 17010201SUPP18. Measures of effectiveness must relate to the performance goals stated in the "Purpose" section of this announcement. Measures of effectiveness must be objective, quantitative and measure the intended outcome of the proposed program. The measures of effectiveness must be included in the application and will be an element of the evaluation of the submitted application. Eligible recipients will be evaluated against the following criteria: ASPR and CDC will evaluate the extent to which the applicant: • Presents outcomes that are consistent with the project period outcomes described in Maximum Points: 35 Approach the ASPR-CDC Project Description and logic model. • Describes an overall strategy and activities consistent with the ASPR-CDC Project Description and logic model. • Describes strategies and activities that are achievable, appropriate to achieve the outcomes of the project, and evidence-based (to the degree practicable). • Shows that the proposed use of funds is an efficient and effective way to implement the strategies and activities and attain the project period outcomes. • Presents a work plan that is aligned with the strategies/activities, outcomes, and performance measures in the approach and is consistent with the content and format proposed by ASPR and CDC. ASPR and CDC will evaluate the extent to which the applicant: • Shows/affirms the ability to collect data on the process and outcome performance measures specified by ASPR and CDC in the project description and presented by the applicant in their approach. • Describes clear monitoring and evaluation procedures and how evaluation and performance measurement will be incorporated into planning, implementation, and reporting of project activities. • Describes how performance measurement and evaluation findings will be reported, and used to demonstrate the outcomes of the FOA and for continuous program quality improvement. • Describes how evaluation and performance measurement will contribute to developing an evidence base for programs that lack a strong effectiveness evidence base. • Includes a preliminary Data Management Plan (DMP), if applicable. Application Review Information. 1. EVALUATION CRITERIA‌ The Project Narrative describes what you intend to do with your project and includes the Evaluation Criteria in Sections A-D below. Your application will be reviewed and scored according to the quality of your response to the requirements in Sections A-D.  In developing the Project Narrative section of your application, use these instructions, which have been tailored to this program.  The Project Narrative (Sections A-D) together may be no longer than 25 pages.  You must use the four (4) sections/headings listed below in developing your Project Narrative. You must indicate the Section letter and number in your response, i.e., type “A-1”, “A-2”, etc., before your response to each question. You may not combine two or more questions or refer to another section of the Project Narrative in your response, such as indicating that the response for B.2 is in C.7. Only information included in the appropriate numbered question will be considered by reviewers. Your application will be scored according to how well you address the requirements for each section of the Project Narrative.  The number of points after each heading is the maximum number of points a review committee may assign to that section of your Project Narrative. Although scoring weights are not assigned to individual bullets, each bullet is assessed in deriving the overall Section score. 1. Application Review Information. In order to receive a Cooperative Agreement for establishing an Exchange, States must submit an application, in the required format, no later than the deadline dates. If an applicant does not submit all of the required documents and does not address each of the topics described below, the applicant risks not being awarded a Cooperative Agreement. Factor 1: Program Plan (40 points) This factor will be evaluated by rating the applicant’s approach to accomplishing each of the requirements identified in the funding opportunity description as demonstrated by the following: Demonstrated understanding of the scope, goals, and objectives of the work required and the applicability and clarity of the overall approach. Discussions detailing how each of the requirements will be performed and the appropriateness of all proposed methodologies and analyses. Identification of potential problems and intended solutions. Discussions detailing the criteria used for selecting sites, list of selected sites or locations of sites, and letters of support from each site, if possible. Discussions of curriculum, including samples of the existing curriculum that will be adapted for the program and preliminary outlines and sample agendas for one or more of the educational sessions described in the funding opportunity description. Potential for the success of the proposed program plan to improve healthy behaviors so as to reduce obesity/overweight of the targeted population. B. Factor 2: Management Plan (30 points) The applicant’s staffing, scheduling, and logistics plans will be evaluated for their effectiveness in committing personnel and resources to provide high-quality service and products within the time frames set-forth. This evaluation is based on the following: Realism of the proposed timeline and the personnel and resources assigned to complete each requirement. Appropriateness of the proposed number of hours estimated for each requirement and each staff member. Adequacy of organizational structure. Adequacy of proposed plan to identify and solve potential problems. Adequacy of proposed plan to monitor and report on program progress and ensure effective communication between program staff members and the DHHS/OWH. C. Factor 3: Experience and Commitment of Key Personnel (20 Points) This factor covers the qualifications of key personnel proposed to perform the work assigned to them and the amount of effort estimated for each person. This evaluation is based on the following: Experience, education, and professional credentials of proposed key personnel on similar projects and in related fields (similar projects must convey similarity in topic, dollar value, workload, duration, and complexity).
