SICK LEAVE/LONG TERM DISABILITY. 39.01 (a) Full-Time Who Have Not Yet Attained The Age of Sixty-Five (65)
SICK LEAVE/LONG TERM DISABILITY. (a) The following is effective April 1, 1982. The Hospital will pay seventy-five percent (75%) of the billed premium towards coverage of eligible employees under the long-term disability portion of the plan (HOODIP or an equivalent plan), the employee paying the balance of the billed premium through payroll deduction. For the purpose of transfer to the short-term portion of the disability program, employees on the payroll as of the effective date of the transfer with three (3) months or more of service shall be deemed to have three (3) months of service. For the purpose of transfer to the long-term portion of the disability program, employees on the payroll as of the effective date of the transfer with one (1) year or more of service shall be deemed to have one (1) year of service. Effective the first month following the transfer, the existing sick leave plan shall be terminated and any provision relating to such plan shall be null and void except as to those provisions relating to pay-out of unused sick leave benefits which are specifically dealt with hereinafter. Existing sick leave credits for each employee shall be converted to a sick leave bank to the credit of the employee at the then current per diem rate of pay based on his/her regular straight time hourly rate. The Asick leave bank@ shall be utilized to:
(1) Supplement payment for lost straight time wages on sick leave days under the new program which would otherwise be at less than full wages or no wages and,
(2) Where a pay-out provision existed under the former sick leave plan in the Collective Agreement, pay-out on termination of employment shall be that portion of any unused sick leave dollars under the former conditions relating to pay-out.
(3) Where, as of the effective date of transfer, an employee does not have the required service to qualify for pay-out on termination, his/her existing sick leave credits as of that date shall nevertheless be converted to a sick leave bank in accordance with the foregoing and he/she shall be entitled, on termination, to that portion of any unused sick leave dollars providing he/she subsequently achieves the necessary service to qualify him/her for pay-out under the conditions relating to such pay-out.
(4) When an employee is absent by reason of incapacity or by reason of an accident occurring while on duty and an award is made by Workplace Safety and Insurance Board in respect thereof, such employee shall be entitled to receive the difference between his/...

9.01 The Employer will pay a seventy-five percent (75%) of the billed premium toward coverage of eligible mployees under the long-term disability portion of the plan (HOODIP or an equivalent plan), the employee paying he balance of the billed premium through payroll deduction. Following three (3) months of service with the mployer, employees will qualify for the short-term portion of the disability program. Following one (1) year of ervice with the Employer, employees will qualify for the long-term portion of the disability program. An employee must return for three (3) weeks full employment, not including work re-entry time period, to re -initiate the first 15 weeks of disability.

SICK LEAVE/LONG TERM DISABILITY. The Hospital will assume responsibility for providing and funding a short-term sick leave plan at least equivalent to that described in the Hospitals of Ontario Disability Plan brochure. SICK LEAVE/LONG TERM DISABILITY. Only personal illness or disability as defined in paragraph 7.25 are covered by this leave policy. When an employee becomes eligible for LTD benefits, she/he shall be considered to be placed on a health leave for the remainder of the school year, and her /his position shall be posted, notwithstanding her/his continued eligibility for LTD benefits. The employee shall notify the Human Resources department not later than April 30 of her/his intent to return to work for the subsequent school year. Such return shall be subject to all other relevant provisions of the agreement. In the event that the employee is not able to return to work at the commencement of the next school year, an additional year’s health shall be granted. In no case shall such leaves extend beyond four years in duration.
SICK LEAVE/LONG TERM DISABILITY. Part-time nurses returning to work from an illness or injury compensable under Workers' Compensation will be assigned light work as necessary, if available. A nurse who transfers from full-time to part-time may elect to retain her accumulated sick leave credits to be during part-time or subsequent full-time employment as provided under the sick leave plan in which she participates as of October The following provision designating regular hours on a daily tour and regular daily tours over the nursing schedule determined by the Hospital shall not be construed to be a guarantee of the hours of work to be performed on each tour or during each tour schedule. Subject to Article below: Where nurses are now working a longer daily tour, the provisions set out in this article governing the regular hours of work on a daily tour shall be adjusted accordingly. The scheduling of meal and relief periods shall be determined by local negotiations. The introduction or discontinuance of longer daily tours shall be determined by local negotiations. NOTE: Hospitals that have extended tours that are presently different than the above, shall maintain those conditions.
SICK LEAVE/LONG TERM DISABILITY. Emergency After Hours Workers The following Sick Leave Plan is intended to provide Emergency After Hours Workers with sick leave coverage for temporary disablement due to illness or non-compensable accidents as well as coverage for long term disability. In determining the entitlement for sick leave hereunder, the Society shall take into consideration illness other than purely physical illness which: • is supported by medical documentation from a legally qualified medical practitioner; and, • directly impacts on an employee’s performance of job duties.
SICK LEAVE/LONG TERM DISABILITY. 5:01 Full Salary for eighty-five (85) eight hour days per year.
(a) Sick Leave balances to be returned to eighty-five (85) eight hour days in the following circumstances:
(i) On January 1st each year,
(ii) When a member returns to duty from long term disability.
(iii) When a member returns from being on short term disability for at least twenty (20) consecutive eight hour days.
(b) If a member is absent for three (3) consecutive tours of duty, due to sickness or accident, the Board might require a medical certificate of incapacity due to such illness or accident. Any costs incurred for the medical certificate are to be paid for by the Board.
(c) If a member is absent for three (3) consecutive tours of duty, due to sickness or accident, the Board may direct the member to attend before a physician employed by the Board for the purpose of verifying incapacity.
(d) When a member is required to fulfill the obligations of 5:01(b) or 5:01(c), the provisions of 7:06 (call out), 9:02 (overtime) and 15:02 (mileage) shall apply.
(e) Additionally, each member shall be entitled to have sixty (60) hours transferred from their sick bank to their lieu bank at the end of each calendar year. For each tour of duty or part thereof a member is absent due to sickness or accident, the time hour for hour shall be deducted from the member’s sick bank. Should a member be absent due to sickness or accident for sixty (60) or more hours, there shall be no transfer of hours to the member’s lieu bank.
5:02 The Board will provide a Long Term Disability Insurance Plan for the Association members. The plan will have a benefit cap of 66 2/3% of the member’s salary with a maximum benefit duration of 60 months. The Board will pay one hundred percent (100%) of the premiums. *Members accumulated sick bank shall be paid an equal amount to his/her salary for one-half (1/2) the number of hours standing to his/her credit after credits have been applied up to and including March 31, 2014. Accumulated sick bank to be paid out no later than April 30, 2014.*

SICK LEAVE/LONG TERM DISABILITY. (a) Effective January 1, of each year commencing January 1, 2012, each employee will be issued 96 hours of sick leave credits. The Employer will pay out one hundred percent (100%) of the unused sick leave credits within thirty days of December 31st of each year. New hires in any given year will receive sick leave credits on a pro-rate basis (based on the regular straight hours anticipated to be worked for the balance of the year compared against either 2,080 hours or 2,184 hours, whichever schedule the new employee is to work at the time of hiring). On termination of employment for any reason, the terminated employee shall be paid the sick leave credits earned by the employee at the time of the termination or return to the Employer any sick leave credits taken by the employee but not yet earned at the time of termination on a pro-rata basis (based on the regular straight hours worked compared against either 2,080 hours or 2,184 hours, whichever schedule the terminated employee is working at the time of termination).
(b) To be paid sick-time for any absence on a pm shift an employee must notify the Employer at least three (3) hours prior to the beginning of the employee’s shift of the absence. For the am shift absence, the employee must endeavour to provide at least three hours notice but not less than 1 hour and 30 minutes.
(c) The sick-time plan is designed for legitimate illness of the employee but up to three (3) shifts may be accessed for personal reasons. When a sick leave is claimed, the employee may be required to furnish proof of illness by a medical certificate, and if it is not so furnished, the employee will not be entitled to sick leave pay providing the Employer may also take such other disciplinary action as may be deemed necessary.
(d) The Employer will pay 100 percent (100%) of the billed premium towards coverage of eligible employees under a short-term disability plan within the current group health benefit plan. An employee will be eligible for short-term disability benefits on the 1st calendar date of injury or the 4th calendar day of illness and will receive seventy-five percent (75%) of their regular basic earnings at the time of the occurrence for a period not to exceed thirty (30) weeks.
